dc.description.abstractLa disfunción sexual puede afectar hasta el 70% de las pacientes con esclerosis múltiple (EM) según estudios previos y es un determinante importante de calidad de vida. En Colombia desconocemos los datos sobre los trastornos sexuales en mujeres con EM y es necesario para incluir este aspecto en su atención. Objetivo: Identificar y caracterizar los trastornos de la función sexual femenina en mujeres en edad fértil con diagnóstico de EM el Hospital Universitario Nacional de la ciudad de Bogotá durante el año 2020 utilizando el índice de función sexual femenina (IFSF). Metodología: Estudio observacional analítico de corte transversal. Se incluyeron mujeres en edad reproductiva con diagnóstico de EM según los criterios de McDonald del 2017. Se excluyeron las pacientes que no tuvieron relaciones sexuales en las 4 semanas previas, discapacidad cognitiva, mujeres con historia de histerectomía, embarazadas o aquellas en los primeros 6 meses de posparto o con recaída de la enfermedad en los 3 meses previos. Se utilizó un cuestionario autoadministrado con las variables sociodemográficas y el IFSF, que puntúa diferentes dominios de la función sexual de acuerdo con la frecuencia informada de síntomas sexuales específicos durante el último mes. Se determinó la frecuencia general y específica de cada dominio de disfunción sexual. Resultados: De 145 mujeres elegibles, 73 respondieron al cuestionario y 47 cumplieron con los criterios del estudio. La edad media (DE) fue de 35,6 (6,4) años. La frecuencia global de disfunción sexual fue del 38,3%. La puntuación IFSF en mujeres afectadas y no afectadas fue 21,1 (4,4) y 30,3 (2,1), p<0.001. La frecuencia de disfunción por dominio específico fue 34% para el deseo sexual, 29,8% para el orgasmo, 17% para excitación y satisfacción, 14,9% para el dolor y 8,5% para la lubricación. En el análisis exploratorio no se encontraron asociaciones estadísticamente significativas entre la disfunción sexual y las variables sociodemográficas y clínicas estudiadas. Conclusiones: En las mujeres en edad reproductiva con diagnostico de EM del Hospital Universitario Nacional existe una frecuencia de disfunción sexual cercana a un tercio de la población de estudio, similar a la prevalencia en la población general colombiana. No se encontraron asociaciones con las características sociodemográficas y clínicas estudiadas. Se requieren intervenciones para establecer un plan diagnostico y terapéutico para los trastornos de la función sexual femenina. (Texto tomado de la fuente).
dc.description.abstractSexual dysfunction can affect up to 70% of women with Multiple Sclerosis (MS) and is an important determinant of quality of life. Since sexual dysfunction involves physiologic and cultural determinants, its frequency varies in different regions. In Colombia, the prevalence of sexual dysfunction in the general population of women is approximately 32,97%, but data on sexual disorders in women with MS are non-existent. Objectives: To identify and characterize sexual dysfunction in women with childbearing potential diagnosed with MS using the female sexual function index (FSFI). Methods: This is an analytical observational cross-sectional study. Women with MS according to 2017 McDonald criteria and with childbearing potential were included; patients with cognitive disability, with previous hysterectomy, pregnant or in the 6 months postpartum or with relapse in the previous 3 months were excluded. A self-reported questionnaire with sociodemographic variables and the FSFI was used. This scores different domains of sexual function according to the reported frequency of specific sexual symptoms during the last month. We determined the overall and domain-specific frequency of sexual dysfunction and assessed their relationship with demographic and MS variables using student’s t and X2 tests. Conclusions: Sexual dysfunction is highly frequent in women with MS in a proportion comparable to that of the general population in Colombia. No associations were found with the sociodemographic and clinical characteristics studied. Further research is warranted to determine the associated factors, and its impact in quality of life.
