dc.description.abstractLa infección por el virus del papiloma humano (VPH) es la infección de transmisión sexual más común en Colombia y el mundo. Dependiendo del genotipo viral, el VPH puede causar lesiones benignas como las verrugas, o inducir procesos de transformación maligna, especialmente cuando se trata de genotipos de alto riesgo, que están asociados con diversos tipos de cáncer, incluido el carcinoma de células escamosas de cabeza y cuello (HNSCC, por sus siglas en inglés), particularmente en la región orofaríngea. Aunque tradicionalmente el cáncer orofaríngeo se ha relacionado con el consumo de tabaco y alcohol, en los últimos años, se ha observado un incremento sostenido de casos relacionados con la infección por VPH, asociado a cambios en los comportamientos sexuales y el incremento en la incidencia de infecciones de transmisión sexual. El carcinoma de orofaringe asociado a VPH (VPH+) constituye una entidad biológica y clínicamente distinta, con diferencias en la respuesta inmune, los perfiles moleculares y el pronóstico en comparación carcinoma de orofaringe No asociado a VPH (VPH-), lo que resalta la importancia de identificar biomarcadores transcripcionales diferenciales en tumores asociados a VPH para avanzar en la comprensión de los mecanismos oncogénicos mediados por el virus y contribuir al desarrollo de herramientas diagnósticas y estrategias terapéuticas más específicas. El objetivo de este proyecto fue identificar y validar biomarcadores transcriptómicos asociados a procesos oncogénicos en carcinoma de orofaringe asociado a VPH mediante el análisis bioinformático de datos públicos y su validación experimental en muestras de pacientes colombianos. Contemplamos el análisis de la expresión génica diferencial, la construcción de redes de coexpresión y la evaluación de la relevancia biológica de los genes priorizados con el fin de establecer una base molecular solida que facilite la caracterización de esta enfermedad en el contexto latinoamericano. Nuestros resultados sugieren que el VPH modula la expresión génica y el microambiente inmune en el cáncer orofaríngeo, favoreciendo un perfil con mejor pronóstico, donde ASCL4 se perfila como un biomarcador clave y una potencial diana terapéutica. dc.description.abstractHuman papillomavirus (HPV) infection is the most common sexually transmitted infection in Colombia and worldwide. Depending on the viral genotype, HPV can cause benign lesions such as warts or induce malignant transformation processes, particularly when involving high-risk genotypes, which are associated with various types of cancer, including head and neck squamous cell carcinoma (HNSCC), especially in the oropharyngeal region. Although oropharyngeal cancer has traditionally been associated with tobacco and alcohol consumption, in recent years a sustained increase in HPV-related cases has been observed, linked to changes in sexual behaviors and the rising incidence of sexually transmitted infections. HPV-positive oropharyngeal cancer represents a biologically and clinically distinct entity, with differences in immune response, molecular profiles, and prognosis compared to HPV no associated oropharyngeal carcinoma. This highlights the importance of identifying differential transcriptomic biomarkers in HPV-associated tumors to advance the understanding of virus-mediated oncogenic mechanisms and to contribute to the development of more specific diagnostic tools and therapeutic strategies. This project aims to identify and validate transcriptomic biomarkers associated with oncogenic processes in HPV associated oropharyngeal carcinoma through bioinformatic analysis of public datasets and their experimental validation in samples from Colombian patients. The research includes the analysis of differential gene expression, the construction of gene co-expression networks, and the evaluation of the biological relevance of prioritized genes, to establish a solid molecular basis that facilitates the characterization of this disease in the Latin American context. Our results suggest that HPV modulates both tumor gene expression, and immune microenvironment in oropharyngeal cancer, favoring a better prognostic profile, where ASCL4 emerges as a key biomarker and a potential therapeutic target.eng
