La labor del cuidador del niño en situación de discapacidad crónica y el impacto de la rehabilitación sobre esta labor

INTRODUCCIÓN: Los cuidadores familiares de personas con algún grado de dependencia, presentan cambios heterogéneos en sus vidas, los cuales afectan múltiples campos de forma simultánea, algunas veces positivamente y en otras ocasiones de forma negativa. El objetivo fue evaluar la labor de los cuidadores de niños en situación de discapacidad crónica y el impacto que ha tenido la rehabilitación en esta problemática, con el objetivo de analizar la discapacidad como una condición familiar y evaluar nuestro quehacer como médicos rehabilitadores. MÉTODOS: Estudio cualitativo con diseño fenomenológico. Fueron 20 participantes seleccionados mediante muestreo combinado. Se usó una entrevista semiestructurada, con la posterior triangulación del análisis de medicina de rehabilitación y psicología clínica. RESULTADOS: Los resultados evidencian que los roles laborales, familiares y sociales de los cuidadores se modifican, con diferentes formas de afrontamiento de su nueva condición de cuidador. La salud física y emocional se afecta, encontrando que muchos cuidadores a pesar del paso del tiempo tienen poca adaptación a la situación. El abordaje de rehabilitación tiene un enfoque en el niño, impactando en algunos cuidadores de forma positiva disminuyendo su estrés y en otros de forma negativa empeorando su condición emocional. CONCLUSIÓN: La labor de cuidar un niño con discapacidad crónica impacta de forma negativa en la vida de los cuidadores y se requiere una intervención dirigida no solo al niño sino también al cuidador y la familia, con el fin de lograr una mejor adaptación y adquisición de estrategias de afrontamiento que disminuyan el impacto negativo.Resumen
Abstract. INTRODUCTION: The familiar caregivers of people with some degree of dependence have heterogeneous changes in their lives, this change affect multiple fields simultaneously, sometimes positively and sometimes negatively. The objective was to evaluate the labor of caregivers of children with chronic disabilities and the impact of the rehabilitation in their labor with the aim of analyzing disability as a family status and evaluate our work as rehabilitation physicians. METHODS: A qualitative study with a phenomenological design. We selected 20 caregivers with combined sampling. A semistructured interview was used, with subsequent analysis by rehabilitation medicine and the clinical psychology and we triangulation of the results. RESULTS: The results show that the work role, the family role and the social role of caregivers are modified with different ways of adaptation to their new status. Physical and emotional health is affected, finding that many caregivers had little adaptation to the situation despite the passage of time. The rehabilitation approach is a focus on the child. Some caregivers were positively impact by rehabilitation and they decreased the stress. In other caregivers the rehabilitation had a negative impact in your emotional condition. CONCLUSION: The labor of caring a child with chronic disability negatively impacts the lives of caregivers. The targeted intervention requires not only the child but, it requires interventions in the caregiver and the family, in order to achieve better adaptation and acquisition coping strategies to reduce the negative impact.Palabras clave
