Asociación entre la infección por Helicobacter pylori y la evolución del recuento plaquetario en niños y adolescentes con trombocitopenia inmune primaria crónica

Introducción: La infección por Helicobacter pylori (Hp) afecta a más de la mitad de la población mundial y se adquiere en la infancia. Su asociación con Trombocitopenia Inmune Primaria (PTI) crónica ha sido demostrada en adultos, sin embargo, existen pocos estudios y resultados no consistentes en niños, especialmente en PTI crónica (PTIc). Objetivos: Determinar si hay una asociación significativa entre la infección por H. pylori y la cronicidad de la PTI en niños y adolescentes. Metodología: Estudio de cohorte prospectivo en pacientes menores de 18 años con PTIc a quienes se les hizo estudio para infección por Hp, el diagnóstico se realizó por una prueba rápida de detección de antígenos en materia fecal. Los pacientes positivos se asignaron aleatoriamente a tratamiento, se hizo seguimiento del recuento plaquetario. Este estudio se proyectó a 18 meses pero por problemas de financiación solo se ha hecho el 50%, el presente es un informe preliminar. Resultados: Se incluyeron 17 pacientes, edad promedio 7 años, 53% sexo masculino. Se identificaron 5 pacientes infectados (29%); la prueba fue negativa en 12 pacientes. La mediana del recuento basal de plaquetas de los pacientes con prueba positiva fue 19.300/mm3 y de 45.000/mm3 para los pacientes con prueba negativa. Después de dar tratamiento a todos los pacientes con prueba positiva, la mediana del recuento plaquetario al tercer mes fue de 39.000/mm3 y la de los pacientes con prueba negativa es de 85.150/mm3 . La erradicación de la infección fue verificada en 2/5 casos, en 1 de ellos se logró respuesta completa; en 2/5 no hubo erradicación después del tratamiento, al tercer mes de seguimiento 1 persiste con trombocitopenia y el otro presenta respuesta completa; hay un paciente pendiente de evaluar.Resumen
Abstract. Introduction: Infection with Helicobacter pylori (Hp) affects more than half of the world population and is acquired in childhood. His association with Primary Immune Thrombocytopenia (ITP) Chronic has been demonstrated in adults, however, few studies and inconsistent results in children, especially in chronic ITP (cITP). Objectives: To determine if there is a significant association between H. pylori infection and chronicity of ITP in children and adolescents. Methods: Prospective cohort study in patients under 18 years cITP were asked to study for Hp infection, the diagnosis was made by a rapid test for detection of antigens in stool. Positive patients were randomized to treatment, monitoring of platelet count was made. This study was designed to 18 months but funding problems only made 50%, this is a preliminary report. Results: 17 patients, mean age 7 years, 53% men were included. 5 infected patients (29%) were identified; the test was negative in 12 patients. The median baseline platelet count of patients with positive test was 19,300 / mm3 and 45,000 / mm3 for patients with negative test. After giving treatment to all patients with a positive test, the median platelet count was the third month 39,000 / mm3 and patients with negative test is 85,150 / mm3. The eradication of the infection was verified in 2/5 cases, 1 of which complete response was achieved; in 5.2 there was no eradication after treatment, monitoring at month 1 persists with thrombocytopenia and the other has complete response; there is a pending assess patient. Conclusion: more patients are required to consider the possible association between H. pylori infection in children and cITP; is not enough information available on this work to suggest that eradication of H. pylori in children with cITP could improve disease.Palabras clave
