Objetivo: Determinar la confiabilidad de la estimación del peso fetal por método clínico de Johnson y Toshach con el peso neonatal inmediato, en mujeres con finalización de la gestación por cualquier vía con embarazos a término o pretermino viable. Pacientes y método: estudio observacional de concordancia diagnóstica y corte transversal. La población correspondió a las mujeres gestantes y sus recién nacidos por cesárea o por parto vaginal, atendidos en un hospital de la red pública de la ciudad de Bogotá. Se incluyeron 137 pacientes con embarazo a término o pretermino viable y se calculo el peso fetal estimado teniendo en cuenta la fórmula de Johnson y Toshach. Se realizó el análisis de concordancia entre el peso fetal estimado por la fórmula de Johnson y Toshach y el peso neonatal inmediato, mediante el coeficiente de correlación – concordancia de Lin y los límites de acuerdo del 95% de Bland-Altman. Se realizó una regresión lineal para ajustar el peso fetal en embarazos pretermino. Resultados: El coeficiente de correlación concordancia entre el peso fetal estimado por el método de Johnson y Toshach y el peso neonatal inmediato en embarazos a término fue de 0.62 (IC 95% 0.53 – 0.70). La estimación del peso fetal por el método clínico de Johnson y Toshach no se modificó ni tuvo cambios relevantes al realizar el análisis de acuerdo al IMC final, el estado de las membranas ni la paridad. Se encontró que tiende a sobreestimar el peso fetal en embarazos pretérminos. Se realizó un ajuste de la fórmula de Johnson y Toshach para esta edad gestacional. Conclusiones: La predicción del peso fetal mediante la técnica de Johnson y Toshach constituye un método confiable, no invasivo, de fácil aplicación y con un coeficiente de correlación concordancia moderado para predecir el peso neonatal inmediato en embarazos a término. Se propone una formula ajustada del método clínico de Johnson y Toshach para embarazos pretermino.Summary
Abstract. Summary: To determine the reliability of the estimation of fetal weight by the clinical method of Johnson and Toshach with the immediate neonatal weight, regardless of the delivery method in women with viable term and preterm pregnancies. Patients and methods: Observational study of diagnostic agreement, transverse and longitudinal section. The population corresponded to pregnant women and their newborns, delivered by cesarean section or spontaneous vaginal delivery, who were treated at a public hospital in Bogotá, Colombia. 137 patient with viable term and preterm pregnancies were included, in which estimated fetal weight was calculated using the Johnson and Toshach's formula. Concordance analysis was performed between the estimated fetal weight obtained with the Johnson and Toshach's formula and the immediate neonatal weight, using the correlation coefficient - concordance of Lin and the limits according to the 95% of Bland-Altman. A linear regression was performed to adjust the fetal weight in preterm pregnancies. Results: The concordance correlation coefficient between the estimated fetal weight by the method of Johnson and Toshach and the immediate fetal weight in term pregnancies was 0.62 (95% CI 0.53 to 0.70). The estimation of fetal weight by the method of Johnson and Toshach was not modified nor had significant changes during the analysis according to the final BMI, membrane status and parity. It was found that this formula tends to overestimate the fetal weight in preterm pregnancies and an adjustment to the Johnson and Toshach formula was performed for this gestational age. Conclusions: The prediction of fetal weight by Johnson and Toshach technique is a reliable, noninvasive and easy to apply method, and it counts with a moderate concordance correlation coefficient for predicting immediate birth weight in term pregnancies. We propose a formula for the adjustment of the clinical method of Johnson and Toshach for the estimation of fetal weight in preterm pregnancies.Keywords
