Caracterización anatómica de la inserción del músculo pterigoideo lateral respecto a la articulación temporomandibular

Conocer la anatomía de la ATM y de las estructuras adyacentes relacionadas con la función del sistema estomatognático, es de vital importancia para el cirujano maxilofacial. El objetivo de este estudio fue caracterizar anatómicamente el musculo pterigoideo lateral respecto a los componentes de la articulación temporomandibular: cóndilo mandibular, menisco y capsula articular, en especímenes cadavéricos humanos colombianos. Se tomó como muestra 18 articulaciones temporomandibulares, correspondientes a 9 cadáveres, masculinos con edad promedio de 60 años, en los que se practicaron disecciones anatómicas siguiendo protocolo previamente diseñado. 88% de la muestra presentó 2 porciones del MPL (superior e inferior), 2% de la muestra presentó 3 porciones del MPL (superior – inferior – accesoria). La porción superior del MPL presento un grosor de 8.7mm, la porción inferior del MPL presento un grosor de 18.5mm y la porción accesoria del MPL presento un grosor de 5mm. La porción superior presento en su mayoría inserción en capsula, disco articular y cóndilo y solo una parte pequeña de la muestra presento inserción únicamente en cápsula y cabeza condilar. La porción inferior por su parte presento inserción en cóndilo mandibular únicamente en toda la muestra. No es posible establecer relación entre características anatómicas del MPL y la presentación clínica de DTM.Resumen
Abstract: Knowing the anatomy of the TMJ and the adjacent structures related to the function of the stomatognathic system is of vital importance for the maxillofacial surgeon. The objective of this study was to anatomically characterize the lateral pterygoid muscle with respect to the components of the temporomandibular joint: mandibular condyle, meniscus and joint capsule, in Colombian human cadaveric specimens. It is a sample of 18 temporomandibular joints, corresponding to 9 corpses, male with an average age of 60 years, in which post-conception practices were previously designed. 88% of the sample presented 2 portions of the MPL (upper and lower), 2% of the sample presented 3 portions of the MPL (upper - lower - accessory). The upper portion of the MPL has a thickness of 8.7 mm, the lower portion of the MPL has a thickness of 18.5 mm and the accessory portion of the MPL has a thickness of 5 mm. The upper portion presents mostly insertion in capsule, articular disc and condyle and only a small part of the sample presents only capsule and condylar head insertion. The lower portion on its part presents the insertion in the lower condyle only in the sample.
