Toxicidad asociada al tratamiento de niños con diagnóstico de leucemia mieloide aguda atendidos en un centro de referencia en Colombia

Antecedentes: La Leucemia Mieloide Aguda (LMA) es un grupo heterogéneo de enfermedades hematológicas malignas, representan el 25% de las leucemias en pediatría. Hay poca información local sobre toxicidad asociada al tratamiento y desenlaces. Objetivo: Describir la toxicidad y complicaciones asociadas al tratamiento de la LMA con el protocolo de tratamiento institucional utilizado en Fundación Hospital Pediátrico la Misericordia. Materiales y métodos: Estudio de cohorte descriptivo-retrospectivo de 33 pacientes menores de 18 años, con diagnóstico confirmado de LMA desde el 1 de Enero del 2012 hasta el 30 de Junio de 2016. Se incluyeron todos los pacientes que cumplían los criterios de inclusión. Resultados: Murieron 17 de 33 pacientes (51,5%). Cinco pacientes murieron en inducción (15,15%), dos (6.06%) tuvieron muerte relacionada con el tratamiento. Un paciente abandono posterior a la segunda inducción. Se presentó 1 abandono (3,03%) y 8 recaídas (28,57%). Todos los pacientes presentaron algún tipo de evento adverso durante el tratamiento, siendo la neutropenia febril la de mayor frecuencia. Se requirió soporte transfusional en todas las fases del tratamiento. Fueron a trasplante alogénico 14 pacientes. La sobrevida global y libre de eventos fue del 42% y 36% respectivamente. Conclusiones: La implementación del protocolo institucional para el manejo de LMA basado en la Guía de Práctica del Ministerio de Salud y Colciencias, ha permitido mejorar significativamente las cifras de sobrevida global y sobrevida libre de eventos al compararlas con los pocos datos existentes y publicados del tema en el país.Resumen
Abstract: Background: Acute Myeloid Leukemia (AML) is a heterogeneous group of malignant hematologic diseases that represent 25% of leukemias in pediatrics. There´s little local information on the toxicity associated to the treatment and outcomes. Objective: Describe the toxicity and complications associated to the treatment of AML with the institutional protocol of treatment used in the Pediatric Hospital “La Misericordia”. Materials and methods: Descriptive-retrospective study of 33 patients under 18 years old, all with confirmed diagnosis of AML from January 1st of 2012 to June 30th of 2016. All patients that met the criteria of inclusion were in fact, included. Results: 17 of the 33 patients deceased (51.5%). 5 deceased during the induction (15.15%), 2 had deaths related to the treatment (6,06%). One patient abandoned the treatment prior to the second induction (3,03%) and eight relapsed. All patients presented some type of adverse sign during treatment, being febrile neutropenia the most frequent one. Transfusion support was needed all the phases of the treatment. 14 patients were moved to allogeneic transplant. The overall and free survival rates were 42% and 36% respectively. Conclusions: The implementation of the institutional protocol for ha doing AML based on the practice Guide of the Ministry of Health and Colciencias, has allowed to significantly improve the overall and free survival rates when compared to the scarcely published and existing data in the country.Palabras clave
