Factores predictores en tomografía computarizada de necesidad de cirugía en pacientes con obstrucción del intestino delgado: Estudio de cohorte (mayo de 2016 hasta mayo de 2018)

Resumen Antecedentes. La obstrucción del intestino delgado es una de las causas de abdomen agudo. El manejo quirúrgico suele reservarse para los casos que no responden al tratamiento conservador, sin embargo, esto puede conducir a una mayor tasa de complicaciones. Algunos estudios sugieren que existen hallazgos de la tomografía que permiten predecir la necesidad de cirugía. Objetivo. Identificar los hallazgos en la tomografía computarizada de abdomen predictores de la necesidad de intervención quirúrgica, en los pacientes con obstrucción del intestino delgado del hospital Universitario Nacional en el periodo comprendido entre el 1 mayo de 2016 y el 30 de mayo de 2018. Métodos y materiales. Se realizó un estudio observacional analítico retrospectivo tipo cohorte. Se incluyeron 55 pacientes adultos con obstrucción del intestino delgado que ingresaron al Hospital Universitario Nacional de Colombia. Se realizó un análisis estadístico descriptivo, bivariado y regresión logística multivariada. Se estimó el OR, la sensibilidad y la especificidad de los hallazgos de la tomografía para predecir la necesidad de cirugía. Resultados. Se incluyeron 55 pacientes, 22 (40%) mujeres. La edad promedio de los pacientes incluidos fue de 51,75 +/- 17,8 años. La mediana de cirugías previas fue significativamente superior en las mujeres (mujeres 2, hombres 1, p=0,009). La presencia de dolor abdominal fue el síntoma más frecuente en el 100% de los pacientes, el vómito ocurrió en el 85,5%, otros síntomas mostraron frecuencia menor al 40%. La cirugía fue realizada en el 52,7% de los pacientes. Dentro de los hallazgos tomográficos, el edema mesentérico mostró asociación con significancia estadística, OR 5,13 (IC95% 1,41 – 18,66), con una especificidad de 84,6% y una sensibilidad de 48,3%. Conclusión. El edema mesentérico en tomografía predice la necesidad de cirugía en pacientes con obstrucción del intestino delgado con una buena especificidad y baja sensibilidad. Adicionalmente, mostró un desempeño aceptable para predecir la necesidad de reintervención y buena capacidad predictiva para la presentación de complicaciones.Resumen
Abstract: Background. The obstruction of the small intestine is one of the causes of acute abdomen. Surgical management is usually reserved in cases that do not respond to conservative treatment, however, this can lead to a higher rate of complications. Some studies suggest that tomography findings exist that allow predicting the need for surgery. Objective. To identify the findings in the computed tomography of the abdomen that predict the need for surgical intervention in patients with small bowel obstruction of the National University Hospital in the period between May 1, 2016 and May 30, 2018. Methodology. The accomplishment of an ambitious cohort analytical observational study is proposed. We will include 55 adult patients with small bowel obstruction treated at the University Hospital of the National University of Colombia. Descriptive statistics techniques will be applied, followed by bivariate analysis and logistic regression (multivariate analysis). OR, sensitivity and specificity of tomography findings that will predict the need for surgery will be estimated. Results. A total of 55 patients were included, 22 (40%) of the cases were women. The average age of the patients included was 51.75 +/- 17.8 years. The median of previous surgeries was significantly higher in women (women 2, men 1, p = 0.009). The presence of abdominal pain was the most frequent symptom, 100% of the patients presented it. Vomiting occurred in 85.5%, other symptoms showed frequency less than 40%. Surgery as a treatment was performed in 52.7% of the cases. Among the findings at the tomography, Conclusion. The identification of mesenteric edema has a specificity of 84.6% to determine the need for surgery; a sensitivity is 71.4% and 75% specificity to predict the need for reoperation; and a sensitivity is 85.7% and specificity 77.1% for the presentation of complications.Palabras clave
