Factores de riesgo intraparto para hemorragia obstétrica severa luego de parto vaginal

dc.description.abstract | OBJETIVO: Identificar los factores de riesgo intraparto para el desarrollo de hemorragia obstétrica en pacientes llevadas a parto vaginal. METODOLOGÍA: Estudio observacional de cohorte retrospectivo, el cual analizó a la población que ingresó a una clínica de alta complejidad de Bogotá, Colombia, para la atención del parto entre Enero y Mayo del 2018.Se incluyeron todas las mujeres con nacimiento por vía vaginal, excluyendo las gestantes con riesgo médico anteparto para hemorragia y cesárea por cualquier indicación. El desenlace principal fue hemorragia obstétrica con activación de código rojo. Se realizó análisis uni y bivariado para identificar el riesgo asociado a cada factor. RESULTADOS: Se evaluaron 702 pacientes con parto vaginal, encontrando 24 casos de hemorragia postparto (incidencia 3,4 %). 165/702 (23,5%) pacientes tuvieron factores de riesgo conocidos para hemorragia postparto. Los factores de riesgo significativos fueron la prolongación de la fase activa del trabajo de parto (RR 4,12 (IC 95% 1,67 - 10,16)), el alumbramiento incompleto con un (RR 5,41 (IC 95% 1,99 - 14,67)) y macrosomía fetal (RR 15,2 (IC 3,59 - 64,42)). CONCLUSIONES: El 87.5% de las hemorragias postparto ocurrieron en mujeres sin factores de riesgo. La prolongación de la fase activa del trabajo de parto, el alumbramiento incompleto o la presencia de un recién nacido macrosómico son factores de riesgo significativos para hemorragia posparto. dc.description.abstract | Abstract: OBJECTIVE: To identify intrapartum risk factors that can lead to obstetric hemorrhage that merits activation of red code in patients undergoing vaginal delivery. METHODOLOGY: An observational retrospective cohort study, which analyzed the population who entered to the highly complex clinic in Bogotá, Colombia to receive attention regarding their labor between January and May of 2018. All women with vaginal birth were included, excluding the pregnant women with medical risk antepartum to hemorrhage and cesarean section, regardless of their indication. The main outcome was obstetric hemorrhage with activation of red code. Uni and bivariate analyzes were performed to identify the risk associated with each factor. RESULTS: We evaluated 702 patients with vaginal delivery, finding 24 cases of postpartum hemorrhage (incidence of 3,4%). 165/702 (23,5%) patients had known risk factors for postpartum hemorrhage. The significant risk factors were the prolongation of the active phase of labor (RR 4.12 (95% CI 1.67 - 10.16)), the incomplete delivery of the placenta (RR 5.41 (95% CI 1 , 99-14.67)) and fetal macrosomia (RR 15.2 (IC 3.59-64.42)). CONCLUSION: 87.5% of postpartum hemorrhages occured in patients without risk factors. The prolongation of the active phase of labor, incomplete delivery of the placenta or the presence of a macrosomic newborn are significant risk factors for a postpartum hemorrhage.
