dc.description.abstract | OBJETIVO: Determinar los factores para el éxito anatómicos y audiométricos de la timpanoplastia tipo I en la población pediátrica del Hospital Fundación de la Misericordia DISEÑO: Observacional analítico de cohorte Ambiespectivo METODOLOGIA: Se realiza la para la parte retrospectiva revisión de Historias clínicas de los pacientes entre los 0 – los 18 años, a quienes se realizó timpanoplastia tipo I del 30 de julio 2006 – 30 julio 2016. Y para la parte prospectiva seguimiento de Pacientes entre los 0 – los 18 años, que iban a ser llevados a Timpanoplastia tipo I de 01 Agosto 2016 - febrero 2018, a estos pacientes se les realizo un seguimiento mínimo de 6 meses donde se evaluó cierre de la perforación timpánica y los resultados audiológicos. Se definió como éxito Neotimpano integro mínimo 6 meses portquirurgicos y Ganancia con GAP 10 dB en audiometría. Se calculo un tamaño de muestra de 78 participantes para un OR a detectar de 2, una significancia de 0.05, un poder de 80% . Se realizara una regresión logística múltiple con metodología hacia atrás y selección de variables por utilidad. Evaluar presencia de interacciones entre las variables incluidas en el modelo. RESULTADOS : Se incluyeron 78 pacientes , se econtro una edad medi de 8,7 años con Rango de 3 – 16 años, 32 mujeres y 46 hombres, éxito cierre de la perforación timapnica fue del 77 %. el éxito cierre de la perforación con respecto al tamaño de la perforación 50% o 50% fue del 78% y 59% respectivamente, no se asocia significativamente P= 0.073. El cierre de perforación con respecto al tipo de injerto utilizado: Tipo Cartílago: 82% y Tipo Fascia: 54%, con una asociación estadísticamente significativo P= 0.01. Éxito cierre perforación por grupo de Edad. El primer grupo de 0 - 5 años: 54%, el segundo grupo de 6 – 10 años: 74%, y el tercer grupo de 11 – 18 años: 68% No se encontró asociación significativa P= 0.322 . Éxito cierre perforación en los pacientes que tenian remanente timpanico fue del 75% , en los pacientes que no tenian remanente timpanico fue del 35% , presenta asociación estadísticamente Significativa P= 0.002. Utilizar injerto de fascia o cartilago en la cirugia no se asocia estadisticamente significativo con éxito audiometrico ya que se encontrol en GAP 10 db. Con una P= 0.14. En el modelo de regresión logística: Tener remanente timpánico da 10 veces mas la probabilidad de tener éxito anatómico comparado con no tener remanente timpánico. P= 0.001. Usar injerto tipo cartílago tiene 6.9 veces mas la probabilidad de tener éxito anatómico comparado con no tener remanente timpánico. P= 0.006 . Usar Injerto tipo Fascia, tiene 0.14 veces probabilidad de cierre de peroración timpánica. P= 0.004. CONCLUSIÓN: Los factores que determinan el mayor éxito en la población pediátrica en timpanoplastia tipo I en nuestro estudio están dados por el uso de injerto tipo cartílago y tener remanente timpánico. Usar injerto de cartílago no afecta el buen resultado audiológico. |
dc.description.abstract | Abstract: OBJECTIVE: To determine the factors for the anatomical and audiometric success of tympanoplasty type I in the pediatric population of the Hospital Fundación de la Misericordia DESIGN: Observational analytical cohort METHODOLOGY: For the retrospective review of histories of patients between 0 - 18 years, who underwent type I tympanoplasty from July 30, 2006 - July 30, 2016. The prospective follow-up of patients between 0 - 18 years, who were scheduled for Type I Tympanoplasty from August 1, 2016 - February 2018. These patients were followed at least for 6 months. The closure of the perforation was evaluated with Tympanic and audiological results. Success was defined as a complete membrane 6 months postoperative and GAP of less than 10 dB in audiometry. A sample size of 78 participants was calculated for an OR to detect of 2, a significance of 0.05, a power of 80%. Multiple logistic regression with backward methodology and selection of variables by utility will be carried out. Evaluate the presence of interactions between the variables included in the model. RESULTS: 78 patients were included, an average age of 8.7 years was counted, with a range of 3 - 16 years, 32 women and 46 men. The success of the closure of the timapnica perforation was 77%. The success of the drilling with respect to the size of the perforation 50% or 50% was 78% and 59% respectively, not significantly related P = 0.073. The closure of the perforation with respect to the type of graft used: Type Cartilage: 82% and Fascia Type: 54%, with a statistically significant association P = 0.01. Successful closing drilling by age group. The first group of 0 - 5 years: 54%, the second group of 6 - 10 years: 74%, and the third group of 11 - 18 years: 68%. No significant relationship was found P = 0.322. Successful closure of perforation in patients who had tympanic remnant was 75%, in patients who had no tympanic remnant was 35%, it presents a statistically significant association P = 0.002. Using the paper and cartilage passage in surgery is not statistically significant with audiometric success and that is in GAP 10 db. With a P = 0.14. In the logistic regression model: Having tympanic time 10 times more than the probability of having anatomical success compared to not having tympanic time. P = 0.001. Using the type of cartilage has 6.9 times more than the probability of having an anatomic success compared to a tympanic remnant P = 0.006. Using Fascia-type graft, it has 0.14 times the probability of closing tympanic peroration. P = 0.004. CONCLUSION: The factors that determine the greatest success in the pediatric population in tympanoplasty type I in our study are given by the use of cartilage-type graft and having a tympanic remnant. Using cartilage graft does not affect the good audiological result. |
