dc.description.abstract | La estenosis subglótica es una de las principales causas de estridor laríngeo en niños, con una prevalecía del 1 -8% en la población pediátrica general, 1-2% de los pacientes en UCI neonatal y 11% de UCI pediátrica, es importante evaluar el grado de obstrucción para definir la necesidad de manejo quirúrgico y pronóstico Actualmente el diagnóstico se hace bajo anestesia general con broncoscopia, exponiendo a la población pediátrica a un riesgo anestésico y a la espera dentro de un servicio general de salud que debe priorizar las salas de cirugía para quienes más lo requieran. Dada la poca correlación clínica con los hallazgos en la broncoscopia, esta priorización usualmente llega a ser errada. Se usa actualmente radiografía simple para apoyar el criterio antes de pasarlo a cirugía, pero esta técnica ha demostrado no ser lo suficientemente específica y sensible para reemplazar la broncoscopia. Actualmente no existe mejor alternativa diagnóstica para la estenosis subglótica que la observación directa a través de la broncoscopia, en el presente estudio se propone la ultrasonografia como método diagnóstico para determinar las emergencias vitales y así priorizar las salas de cirugía para los pacientes que se haya comprobado que así lo requieren, pero el actual estudio demuestra que no es una herramienta diagnóstica adecuada |
dc.description.abstract | Objectives: Subglottic stenosis represents one of the principal causes of laryngeal stridor in pediatric population and the gold standard for diagnose it es bronchoscopy, this procedure is realized under general anesthesia exposing the patient to all the risks general anesthesia comes with and the waiting time within a general health service who should prioritize the operative rooms for those who require it most. Ultrasonography has been positioned several years ago as an accurate and useful technique in the evaluation of subglottic stenosis in children, so it is intended to propose this technique as a diagnostic tool for subglottic stenosis Methods: This is a prospective analytical primary study, an airway ultrasound was requested for all patients under 14 years old with subglottic stenosis diagnosis suspected, and the results were compared with findings at the bronchoscopy. Results: Airway ultrasonography compared with bronchoscopy, as the Gold standard for subglottic stenosis diagnosis according to this study has a sensitivity of 46.6% and a specificity of 73.81%. Conclusion: Ultrasound is not an adequate diagnostic method to determine the degree of subglottic stenosis |
