dc.description.abstract | Introduction: The technique failure of the peritoneal dialysis (PD) is an unwanted event because it occurs in an unplanned manner and, therefore, there is no patient preparation for the change in dialysis modality or the early completion of a permanent vascular access. In addition, the change from PD to HD affects the patient's daily routine, increases health costs, and increases hospitalization rates. To Know the factors associated with the technique failure would help to understand this outcome and improve clinical outcomes. Objective: Identify the factors associated with the time of the technique failure in patients incident to PD during the first year of follow-up. Participants: Patients older than 18 years with diagnosis of CKD in terminal stage with peritoneal dialysis treated in the RTS clinics in Colombia between January 1th, 2016 and December 31th, 2017. Methodology: A retrospective cohort study, the association between a group of independent variables and the technique failure of the peritoneal dialysis was evaluated using Cox regression and adjusting for baseline variables. Results: 2210 patients started peritoneal dialysis; the incidence of technique failure was 8.8 x 100 person-years. The factors that decrease the risk of technique failure were Kt / V> 1.7 (HR: 0.41; p = 0.000) and albumin (HR: 0.61; P = 0.007). The Arterial hypertension increase the risk of technique failure 1.59 times more (HR: 1.59; p=0.044) and male sex increases the risk of technique failure 1.62 times more (HR: 1.62; 0 = 0.038). Conclusions: This study found that Kt / V> 1.7 and albumin decrease the risk of presenting technique failure in the first year of follow-up; and the factors that increase the risk of technique failure in the first year of follow-up were: male sex and high blood pressure. |
dc.description.abstract | Introducción: La falla de la técnica en diálisis peritoneal (DP) es un evento no deseado debido a que ocurre de forma no planificada y, por lo tanto, no hay una preparación del paciente al cambio de modalidad de la diálisis ni la realización anticipada de un acceso vascular permanente. Además, el cambio de DP a HD afecta la rutina diaria del paciente, incrementa los costos en salud, y aumenta las tasas de hospitalización. Conocer los factores clínicos asociados a la falla de la técnica ayudaría a comprender este desenlace y a mejorar resultados clínicos. Objetivo: identificar los factores asociados con el tiempo de la falla de la técnica en pacientes incidentes en DP durante el primer año de seguimiento. Participantes: pacientes mayores de 18 años con diagnóstico de ERC en etapa terminal incidentes en DP tratados en las clínicas de RTS en Colombia entre el 1 de enero de 2016 y 31 de diciembre de 2017. Metodología: Estudio de cohorte retrospectiva, se evalúo la asociación entre factores clínicos individuales y el riesgo de presentar falla de la técnica en DP durante el primer año de seguimiento. Resultados: 2210 pacientes iniciaron DP, la tasa de incidencia de falla en toda la cohorte fue 8.8 x 100 personas-año. Los factores que disminuyen el riesgo de falla de la técnica fueron Kt/V >1.7 (HR: 0.41; p=0.000) y albúmina (HR: 0.61; P=0.007). Tener hipertensión arterial aumenta el riesgo de falla de la técnica 1.59 vece más (HR: 1.59; p=0.044) y sexo masculino aumenta el riesgo de falla de la técnica 1.62 veces más (HR: 1.62; 0=0.038). Conclusiones: Este estudio encontró que el Kt/V >1.7 y la albumina disminuyen el riesgo de presentar falla de la técnica en el primer año de seguimiento; y los factores que aumentan el riesgo de falla de la técnica en el primer año de seguimiento fueron: el sexo masculino y tener hipertensión arterial. |
