dc.description.abstractLa Sociedad de medicina Submarina e Hiperbárica de norteamérica (Undersea and Hyperbaric Medical Society) define a la oxigenoterapia hiperbárica (HBOT, por sus siglas en inglés) como una intervención en la cual se le administra a un individuo, oxígeno al 100% de concentración, intermitentemente mientras está en una cámara a presiones mayores a una atmósfera y tiene 14 usos aceptados para dicha terapia, entre estos el tratamiento de quemaduras térmicas, gangrena gaseosa y posterior a realización de injertos y colgajos. Se realzó una revisión sistemática que comprendiera la evidencia para su uso en estas 3 indicaciones a partir del año 2000, se identificaron 234 artículos que inicialmente de los cuales al final solo 10 cumplieron los criterios de inclusión. Se evaluó la calidad de la evidencia usando la metodología Grade con la herramienta Grade Pro, obteniendo como resultado una evidencia moderada con recomendación a favor para el uso de la HBOT para la prevencion de necrosis en colgajos y supervivencia de injertos, calidad de evidencia alta sin beneficio adicional a favor de la HBOT para el tratamiento de quemaduras térmicas y evidencia muy baja para algún tipo de recomendación en el manejo de gangrena gaseosa. Se concluye que la evidencia en la cual está soportada el uso de la HBOT proviene mayoritariamente de estudios previos a las fechas seleccionadas para la presente revisión sistemática, se requieren nuevos estudios metodológicamente adecuados para actualizar la evidencia y las recomednaciones de la HBOT como terapia coadyuvante en los 3 escenarios clínicos planteados en la presente revisión.
dc.description.abstractThe Undersea and Hyperbaric Medical Society (UHMS) defines hyperbaric oxygen therapy (HBOT) as an intervention in which an individual is administered oxygen at 100% concentration, intermittently while in a chamber at pressures greater than 1.0 atmospheres and there are 14 accepted uses for such therapy, including the treatment of thermal burns, gas gangrene, and skin grafts and free flaps. A systematic review was conducted that included the evidence for its use in these 3 indications from the year 2000, 234 articles were identified initially, only 10 of them met the inclusion criteria at the end. The quality of the evidence was evaluated using the Grade methodology with the Grade Pro tool, obtaining as a result moderate evidence with a favorable recommendation for the use of HBOT in the prevention of necrosis in free flaps and skin grafts survival, high quality of evidence without additional benefit in favor of HBOT for the treatment of thermal burns and very low evidence for any type of recommendation in the management of gas gangrene. It is concluded that the evidence in which the use of HBOT is supported comes mostly from studies prior to the dates selected for this systematic review, new adequate methodologically studies are needed to update the evidence and grade of recommendation of HBOT as adjuvant therapy in the 3 clinical scenarios proposed in this review.
