dc.description.abstract | Since 1884 the cause for venous thromboembolic disease has been described as the formation of thrombi and its embolism and includes deep vein thrombosis and pulmonary thromboembolism (PE). Surgical procedures, regardless of the speciality that performs them, have been considered as a risk factor for the development of venous thromboembolism due to endothelial injury and the stasis that occurs during the surgical procedure and the postoperative period. It has been estimated that patients in immediate postoperative have a risk 70 times greater of developing venous thromboembolism than the general population. And in case of developing one, it exposes those who suffer it to greater morbimortality, longer hospital stay and its subsequent increased costs to the Healthcare System. For this reason, scores have been devised to detect the thromboembolism risk to establish prophylactic measures for the prevention of these events. The current score with the greatest support of scientific evidence is the Caprini score. However, it is very detailed thus its length makes it costly and restricts its use. There are shorter scales with the same objective (to determine the thromboembolism risk in surgical patients) albeit with no studies to evaluate the concordance between these scales. This study aims to evaluate the concordance among these scores in order to find the most appropriate scale to evaluate thromboembolism risk in our setting. |
dc.description.abstract | Desde el año de 1884, se describe la formación de trombos y su embolización como la causa de la enfermedad tromboembólica venosa la cual comprende la trombosis venosa profunda y el tromboembolismo pulmonar (TEP). Los procedimientos quirúrgicos, independiente de la especialidad que lo realice, han sido considerados como un factor de riesgo para el desarrollo de tromboembolia venosa, debido a la lesión endotelial y estasis que se produce durante el procedimiento y el período post-operatorio. Se ha estimado que los pacientes en post-operatorio inmediato tienen un riesgo de presentar tromboembolia venosa 70 veces mayor que el de la población general, y en caso de presentarse expone a quien la sufre a una mayor morbimortalidad, mayor estancia hospitalaria, con el consecuente aumento de los costos al sistema de salud. Es por esto que se han desarrollado escalas para la detección de riesgo tromboembólico, con el fin de instaurar medidas profilácticas para la prevención de estos eventos. La escala actualmente con mayor respaldo por la evidencia científica es la escala de Caprini, sin embargo, es muy extensa, por lo tanto, su longitud la vuelve dispendiosa y limita su uso; existen escalas más cortas que tienen el mismo objetivo (determinación del riesgo tromboembólico en pacientes quirúrgicos), sin embargo, hasta el momento no se han realizado estudios en donde se evalúe la concordancia de dichas escalas. El presente estudio tiene como objetivo evaluar la concordancia entre estas escalas para así mismo encontrar la escala más apropiada para la evaluación del riesgo tromboembólico en nuestro medio. |
