Desenlaces clínicos y económicos asociados a cirugía de próstata en el régimen contributivo en Colombia

dc.description.abstract | We compared clinical outcomes and costs in Colombia for three surgical modalities for benign prostatic hyperplasia (BPH): open prostatectomy (OP), transurethral resection of the prostate (TURP), and laser ablation or photovaporization of the prostate. We conducted a retrospective cohort study analyzing data from the contributory system of the general social security system in Colombia, including patients undergoing surgical management of BPH from 2011 to 2015. Results: 40,587 patients were included. The average age was 68 years. Perioperative mortality was 0.32%. The ICU management requirement was 1.68%. The transfusion requirement was 1.6% and the reoperation rate was 5.45%. The risk factors associated with mortality and ICU requirement were age, Charlson index, type of surgery, and geographic region (p <0.005). The average cost for OP was USD $ 789, TURP USD $ 721 and laser techniques USD $ 416. Additionally, a relationship was found between the increase in costs and the age of the patients, the Charlson index and the region of the country, being more expensive in the Pacific area (p <0.005). Conclusion: To our knowledge, this is the only study at the national level that evaluates clinical and economic outcomes for surgical interventions for BPH. The results of TURP and laser ablation were superior in terms of associated complications and costs compared to AP. |
dc.description.abstract | En este estudio comparamos los desenlaces clínicos y los costos en Colombia para tres modalidades quirúrgicas para la hiperplasia prostática benigna (HPB): prostatectomía abierta (PA), la resección transuretral de próstata (RTUP) y la ablación o fotovaporización láser de próstata. Realizamos un estudio de cohorte retrospectiva analizando los datos del sistema contributivo del sistema general de seguridad social en salud de Colombia, incluyendo los pacientes sometidos a manejo quirúrgico de HPB del 2011 al 2015. Se incluyeron 40.587 pacientes. La edad promedio fue de 68 años. La mortalidad perioperatoria fue de 0,32%. El requerimiento de manejo en UCI fue 1,68%. El requerimiento de transfusión fue de 1.6% y la tasa de reintervenciones fue del 5.45%. Los factores de riesgo asociados a mortalidad y al requerimiento de UCI fueron la edad, el índice de Charlson, el tipo de cirugía y la región geográfica (p<0.005). El costo promedio de la PA en dólares fue de USD $789, la RTUP de USD $721 y las técnicas láser de USD $416. Adicionalmente, se encontró una relación entre el aumento de los costos y la edad de los pacientes, el índice de Charlson y la región del país, siendo más costoso en la zona del Pacífico (p<0.005). Para nuestro conocimiento, este es el único estudio a nivel nacional que evalúa desenlaces clínicos y económicos para intervenciones quirúrgicas para HPB. Los resultados de la RTUP y de la ablación por láser fueron superiores en cuanto a complicaciones asociadas y costos respecto a la PA. |
