dc.description.abstractHallux valgus is probably the most frequent complaint in foot and ankle surgery, and multiple surgical techniques have been described for its treatment. However, there is great controversy regarding this subject, since it has not been shown that one technique is superior to the others. Within the therapeutic arsenal available, recently - and thanks to technological advances and improvements in the instruments used for surgery - percutaneous techniques have appeared as a treatment option that reduces trauma to soft tissues. The aim of this study is to retrospectively compare the clinical outcomes of two groups of patients who underwent surgery to correct hallux valgus: a group was taken to conventional open surgery and the other group was taken to a percutaneous procedure. It was noticed that the time to union, the time to return to work and the satisfaction with the procedure was comparable in the two groups. However, the group that was taken to percutaneous osteotomies presented less postoperative pain (OR 7.94, 95% CI: 1.88-33.4) and a lower rate of soft tissue complications (3.7% vs 15 %, OR 0.06, 95% CI: 0.007-0.52). Percutaneous techniques for the correction of hallux valgus are considered to be at least as good as open osteotomies; with similar functional outcomes, a lower rate of soft tissue complications and offering better postoperative pain control.
dc.description.abstractEl hallux valgus es probablemente el motivo de consulta más frecuente en la cirugía de pie y tobillo y para su tratamiento se han descrito múltiples técnicas quirúrgicas. Sin embargo, a día de hoy, existe gran controversia con respecto al tema, puesto que no se ha demostrado que una técnica sea superior a las otras. Dentro del arsenal terapéutico disponible, recientemente – y gracias a los avances tecnológicos y mejoras en el instrumental - han aparecido las técnicas percutáneas como una opción de tratamiento que disminuye el trauma sobre los tejidos blandos. El objetivo del presente trabajo es comparar de forma retrospectiva los desenlaces clínicos de dos grupos llevados a cirugía para corrección de hallux valgus: un grupo con cirugía abierta convencional y otro grupo sometido a procedimientos percutáneos. Se evidenció que el tiempo para consolidación, el tiempo de reintegro laboral y la satisfacción con el procedimiento fue comparable en los dos grupos. Sin embargo, el grupo de las osteotomías percutáneas presentó un mejor control del dolor postoperatorio (OR 7,94, IC 95%: 1,88-33,4) y una menor tasa de complicaciones de tejidos blandos (3,7% vs 15 %, OR 0,06, IC 95%: 0,007-0,52). Se considera que las técnicas percutáneas para la corrección del hallux valgus son, por lo menos, comparables a las técnicas abiertas; con desenlaces similares, menor tasa de complicaciones y ofreciendo un mejor control del dolor postoperatorio.
