Pulmonary hypertension (PH) is a rare pathology, and in the literature, most of the existing evidence focuses on adults, from which data are extrapolated to pediatrics. Within its pathophysiology, it is known that hypobaric hypoxia secondary to high altitude plays an important role in its etiology, especially in children, who can be affected since the prenatal stage. Despite the available treatment, PH implies great morbidity and mortality for the pediatric patient. Given the problem described and considering that there are few biomarkers available in clinical practice with enough evidence for the diagnosis and follow-up of this pathology, Endothelin-1 (ET-1) is highlighted, as a molecule involved in the pathophysiological mechanisms that generate PH and in certain drugs used as the first line of treatment. For this reason, an analytical observational case-control study is proposed, taking into account as cases pediatric patients with pulmonary hypertension who developed the disease at high altitude (> 2500 masl), VS at low altitude (<1000 masl), compared with healthy controls that inhabit both levels. In all the subgroups described, ET-1 will be measured in peripheral venous blood (with prior informed consent) and its levels will be analyzed according to the established study groups and the variables selected to characterize the population and the disease, to provide evidence that supports its usefulness as a diagnostic tool and that contributes to establishing the influence of hypobaric hypoxia on the pathophysiology of the disease.
