dc.description.abstractLa efectiva gestión de las redes de suministro de medicamentos es un elemento clave para la promoción, protección y recuperación de la salud de la población. El Estado, como cabeza del sistema, está llamado a definir estrategias para afrontar las complejidades propias de la operación de las redes, y garantizar principios de universalidad en el acceso y solidaridad. Los programas de salud pública en comunidades apartadas enfrentan retos aún mayores debido a condiciones de dispersión y limitaciones en la infraestructura de acceso y comunicaciones. Esta investigación busca diseñar estrategias de operación logística, basadas en la coordinación de los diferentes actores, que permitan la mejora del desempeño del sistema en términos de accesibilidad, disponibilidad y oportunidad en la prestación de los servicios de salud. A partir de la caracterización de la red de suministro de medicamentos en el Departamento del Chocó, se identificaron oportunidades de mejora en la prestación de los servicios de salud pública y se propuso una estrategia de coordinación en los procesos de aprovisionamiento y transporte, basada en incentivos para las Instituciones Prestadoras de Salud – IPS. La estrategia propuesta es evaluada mediante modelación matemática formal a partir de la información recolectada en el contexto práctico. Los resultados en diferentes escenarios sugieren que la estrategia logra la reducción de los costos en un 28% para el escenario coordinado respecto a la línea base, y brinda incentivos a las IPS para el trabajo conjunto en la búsqueda de los objetivos globales de la red. dc.description.abstractThe effective management of medicine supply networks is a key element for the promotion, protection, and recovery of the population's health. The State, as head of the system, is called upon to define strategies to face the complexities inherent to the operation of the networks and guarantee principles of universality in access and solidarity. Public health programs in remote communities face even greater challenges due to dispersed conditions and limitations in access and communication infrastructure. This research seeks to design logistics operation strategies, based on the coordination of the different actors, that allow the improvement of the performance of the system in terms of accessibility, availability, and opportunity in the provision of health services. Based on the characterization of the drug supply network in the Department of Chocó, opportunities for improvement in the provision of public health services were identified and a coordination strategy based on incentives was proposed in the supply and transportation processes for Health Provider Institutions - IPS. The proposed strategy is evaluated through formal mathematical modeling based on the information collected in the practical context. The results in different scenarios suggest that the strategy achieves a reduction up to 28% in cost for the coordinated scenario compare to the baseline and provides incentives to the IPS to work together in pursuit of the global objectives of the network.
