dc.description.abstract | Introducción Las infecciones musculoesqueléticas son una patología frecuente en la población pediátrica, históricamente causadas por el Staphylococcus Aureus Meticilino-Sensible (SAMS), pero con un creciente aumento en la última década de los casos por Staphylococcus Aureus Meticilino Resistente (SAMR). El diagnóstico temprano, así como un manejo oportuno y adecuado es fundamental en el tratamiento de esta patología. Para esto es crucial identificar lo más pronto posible el agente infeccioso guiando de forma más precisa el manejo antibiótico. Por esto el propósito de este estudio es identificar factores e indicadores de riesgo que puedan detectar la presencia de SAMR de forma temprana en pacientes pediátricos con infecciones musculoesqueléticas. Materiales y métodos Estudio observacional analítico de tipo cohorte retrospectivo en población pediátrica. Se incluyeron dos grupos: Uno con diagnóstico de infecciones musculoesqueléticas por SAMR, y otro por SAMS u otro germen. Evaluando las características clínicas y paraclínicas en cada grupo, para determinar factores que clasifiquen de forma adecuada la presencia o no de SAMR mediante la curva ROC y OR de cada factor asociado. Resultados Se incluyeron 193 pacientes, 97 con cultivo positivo para SAMR. Identificando las variables hematocrito < 34.6%, PCR mayor a 66.5 mg/L, leucocitos > 17135 cel/mm3 y y temperatura >36.4º como factores de riesgo con un OR estadísticamente significativo y con una adecuada curva ROC para identificar la presencia de SAMR. Se encontró una sensibilidad aproximada de 70% y una especificidad cercana al 90%, con una precisión del 80%. Discusión Identificar la presencia de SAMR se traduce en un manejo temprano y la posibilidad de iniciar tratamientos específicos contra este, con un mejor control de la enfermedad y así evitar complicaciones como desenlaces fatales. dc.description.abstract | Introduction Musculoskeletal infection is a common pathology in the pediatric population, historically caused by Methicillin-Sensitive Staphylococcus Aureus (MSSA) but with a significative increase in the last decade of cases due to Methicillin-Resistant Staphylococcus Aureus (MRSA). Early diagnosis, as well as adequate and timely management, is essential in the treatment of this pathology. It is crucial to identify the infectious agent as soon as possible, guiding the antibiotic more precisely. Therefore, the purpose of this study is to identify risk factors that can detect the presence of MRSA in the pediatric population with musculoskeletal infections. Materials and methods We conducted a retrospective Analytical observational study of pediatric patients with musculoskeletal infections. Two groups were included: One with diagnosis of musculoskeletal infection due to MRSA and a second one due to SAMS or another germ. We assessed clinical and paraclinical characteristics in each group, to determine factors that adequately classify the presence or absence of MRSA using the ROC and OR curve of each associated factor. Results 193 patients were included, we identified 97 cases of MRSA. Hematocrit < 34.6%, CRP greater than 66.5 mg/L, leukocytes > 17,153 cells/mm3 and temperature > 36.4ª C were identified as risk factors with a statistically significant OR and with an adequate ROC curve to identify the presence of MRSA. An approximate sensitivity of 70% and a specificity close to 90%, with an accuracy of 80%, were found. Discussion Early recognition of MRSA allows starting appropriately specific treatments against it, with better control of the disease and thus avoiding complications such as fatal outcomes. |
