Supervivencia a 3 años, costos y factores asociados en pacientes mayores de 65 años llevados a colecistectomías en Colombia: una cohorte nacional retrospectiva

dc.description.abstract | Estudio retrospectivo de cohortes basado en datos de reclamaciones administrativas que utiliza la base para el estudio de la suficiencia de la UPC del ministerio de salud y protección social de Colombia y el RUAF para las defunciones. El desenlace principal fue la supervivencia a tres años después de la cirugía, adicionalmente se evaluó mortalidad a 30 días y un año, así como costos globales. Resultados: Se incluyeron un total de 37,095 pacientes que fueron sometidos a colecistectomía laparoscópica (29,797. [80.33%]) y abierta (7,298 [19.6%]) entre los años 2012 y 2017 en Colombia, y distribuidos por grupos etarios. Se encontró mayor mortalidad a 30 días, 1 año y 3 años en los mayores de 90 años (RR 15.6 p 0.000 IC 11.86-20.5) y con mayor índice de Charlson (RR 4.74 p 0.000 IC 3.53-6.38), mayor mortalidad en pacientes sometidos a colecistectomía abierta con respecto a laparoscópica (RR 0.13, p 0.000 IC: 0.14-0.18), sin diferencias en cuanto a mortalidad en el análisis por regiones y sí una mayor supervivencia a 3 años en el Atlántico. Resulta más costosa una colecistectomía en los departamentos remotos de Colombia. La colecistectomía laparoscópica cuando se analizan los costos globales, resulta más económica que el procedimiento abierto. La supervivencia a 3 años fue mayor en los grupos etarios más jóvenes con diferencia estadísticamente significativa, así como también en las mujeres, con diferencia estadísticamente significativa. Conclusión: El presente estudio encontró que la mortalidad asociada a colecistectomía abierta y laparoscópica en Colombia, es similar a la encontrada en la literatura universal; es más costo efectivo la colecistectomía laparoscópica que la abierta, así como que en los departamentos más remotos es más costosa la colecistectomía. Existe mayor supervivencia a 3 años en pacientes más jóvenes y sin comorbilidades. dc.description.abstract | Retrospective cohort study based on data from administrative claims that uses the base for the study of the adequacy of the UPC of the Ministry of Health and Social Protection of Colombia and the RUAF for deaths. The main outcome was 3-year survival after surgery, additionally, 30-day and 1-year mortality was evaluated, as well as global costs. Results: A total of 37,095 patients who underwent laparoscopic (29,797 [80.33%]) and open (7,298 [19.6%]) cholecystectomy between 2012 and 2017 in Colombia, and distributed by age groups, were included. Higher mortality at 30 days, 1 year and 3 years was found in those over 90 years of age (RR 15.6 p 0.000 IC 11.86-20.5) and with a higher Charlson index (RR 4.74 p 0.000 IC 3.53-6.38), higher mortality in patients undergoing open cholecystectomy with respect to laparoscopic (RR 0.13, p 0.000 CI: 0.14-0.18), without differences in terms of mortality in the analysis by regions and a greater 3-year survival in the Atlantic. A cholecystectomy is more expensive in the remote departments of Colombia. Laparoscopic cholecystectomy when global costs are analyzed, is cheaper than the open procedure. The 3-year survival was higher in the younger age groups, with a statistically significant difference, as well as in women, with a statistically significant difference. Conclusion: The present study found that the mortality associated with open and laparoscopic cholecystectomy in Colombia is similar to that found in the universal literature; laparoscopic cholecystectomy is more cost effective than open, as well as cholecystectomy in more remote departments. There is greater survival at 3 years in younger patients and without comorbidities. More evidence and clinical studies are needed to elucidate some of the results of the present study. |
