Caracterización del transporte aeromédico civil en Colombia para los años 2020 y 2021

dc.description.abstract | Introducción: en la actualidad el transporte aeromédico es una operación bien establecida y confiable, que surge como una opción para movilizar pacientes de un lugar remoto o inexequible vía terrestre hasta un sitio de atención médica que supla todas las necesidades que requiera el individuo. Colombia cuenta con una buena cobertura en salud, pero la distribución de los servicios de alta complejidad es centralizada, por lo cual el servicio de transporte aeromédico brinda un apoyo en la garantía de atención oportuna en salud de habitantes de territorios alejados que no cuentan con instituciones de alta complejidad. Dado lo anterior es de interés caracterizar la operación civil de las ambulancias aéreas a lo largo del terreno nacional e identificar las principales patologías de los pacientes transportados; para así contar con una aproximación estadística a la situación actual de los servicios de transporte aeromédico. Metodología: se planteó un estudio de tipo observacional, descriptivo de corte transversal, el cual buscó caracterizar los diversos factores implicados en el transporte aeromédico civil en Colombia en los años 2020 y 2021, se estableció la proporción de patologías clínicas que requirieron ser transportadas por ambulancia aérea y dada la pandemia se realizó un énfasis de la infección por SARS-CoV-2 en Colombia. Resultados: se obtuvo un registro de 13085 pacientes civiles que hicieron uso de ambulancia aérea en Colombia para los años 2020 y 2021; la mediana de minutos para desplazar los pacientes de una ciudad a otra fue de 80 minutos; los departamentos con mayor demanda del servicio (origen) fueron Arauca (13%), Cauca (12,1%), y Chocó (10,6%), en cambio los departamentos con mayor cantidad de pacientes recibidos (destino) fueron Meta (16,5%), Bogotá (15,6%) y Santander (12,6%). La mediana de edad de los pacientes fue de 36 años; los diagnósticos con mayor frecuencia reportados fueron patologías pertenecientes a los grupos del sistema respiratorio (19,5%), seguido por alteraciones gineco obstétricas (13,1%), el sistema cardiovascular (12,4%), y afecciones neonatales (11,4%). Por otra parte, los departamentos clasificados como distantes según la Ley 1122 del 2007 Capitulo 4, Articulo 14-I representaron el 23% de los registros de transporte aeromédico del país. Discusión: el presente estudio ha sido de los pocos en caracterizar el transporte aeromédico durante los primeros dos años de la pandemia y con una casuística tan grande en Latinoamérica; los diagnósticos más frecuentes reportados difieren de lo hallado en la literatura, la cual indica que las patologías con mayor incidencia son del sistema cardiovascular, neurológicas y trauma; en nuestra investigación el contexto de la pandemia por SARS-CoV-2 favoreció una mayor representación de patologías respiratorias transportadas. Basándonos en la información reportada se puede evidenciar la importancia del transporte aeromédico en Colombia y como este brinda un servicio complementario para garantizar la atención de alta complejidad en dichos territorios. dc.description.abstract | Introduction: The aeromedical transport is a well-established and reliable operation, which arises as an option to move patients from a remote or inaccessible place by land to a medical care site that meets all the needs required by the individual. Colombia has good health coverage, but the distribution of highly complex services is centralized, which is why the aeromedical transport service provides support in guaranteeing timely health care for people of remote territories that do not have institutions of high complexity. Therefore, it is of interest to characterize the civil operation of air ambulances throughout the national terrain and to identify the main pathologies of the transported patients; to have a statistical approximation to the current situation of aeromedical transport services. Methodology: It was an observational, descriptive cross-sectional study, which sought to characterize the various factors involved in civil aeromedical transport in Colombia in the years 2020 and 2021, establishing the proportion of clinical pathologies that required being transported by air ambulance and due the pandemic, it had an emphasis on the infection by SARS-CoV-2 in Colombia. Results: a total of 13,085 civilian patients who used the air ambulance in Colombia for the years 2020 and 2021; the median time to move patients from one city to another was 80 minutes; The departments with the greatest demand for the service (origin) were Arauca (13%), Cauca (12.1%), and Chocó (10.6%), while the departments with the greatest number of patients received (destination) were Meta ( 16.5%), Bogota (15.6%) and Santander (12.6%). The median age of the patients was 36 years; The most frequently reported diagnoses were pathologies belonging to the respiratory system groups (19.5%), followed by gynecologic-obstetric disorders (13.1%), the cardiovascular system (12.4%), and neonatal conditions (11, 4%); the departments classified as distant according to Law 1122 of 2007 Chapter 4, Article 14-I represented 23% of the country's aeromedical transport records. Discussion: the present study has been one of the few to characterize aeromedical transport during the first two years of the pandemic and with such a large number of cases in Latin America; the most frequent diagnoses reported differ from what is found in the literature, which indicates that the pathologies with the highest incidence are of the cardiovascular system, neurological and trauma; In our research, the context of the SARS-CoV-2 pandemic favored a greater representation of transported respiratory pathologies. Based on the information reported, the importance of aeromedical transport in Colombia can be evidenced and how it provides a complementary service to guarantee highly complex care in those distant territories. |
