Configuración de arcos parciales VMAT para tratamiento de radioterapia de mama izquierda, fraccionamiento FAST FORWARD

La radioterapia adyuvante para el cáncer de mama es un tratamiento estándar que se utiliza para mejorar el control tumoral local y la supervivencia general. Para América Latina y el Caribe, se estima un aumento en la incidencia de casos nuevos del 47% para el año 2040. [1] Este estudio utiliza los criterios dosimétricos de aceptación del ensayo UK-FAST-FORWARD (26 Gy – 5 Fx), con el fin de evaluar la técnica de tratamiento VMAT (Arcoterapia de Intensidad Modulada Volumétrica) en comparación con las técnicas generalmente usadas IMRT (Radioterapia de Intensidad Modulada) y 3DCRT (Radioterapia conformal en 3D), para una muestra aleatoria de 20 pacientes. Es importante señalar que estos criterios dosimétricos se aplicarán a todas las técnicas mencionadas, incluyendo VMAT, IMRT y 3DCRT. El software de análisis de ciencia de datos llamado STATA se utilizó para verificar que las diferencias observadas en las muestras poseen significancia estadística. Este análisis mostró una mejora significativa (p<0.05) de los índices de conformidad para las técnicas IMRT y VMAT en comparación con técnica conformal. No se observaron diferencias estadísticamente significativas para el índice de homogeneidad entre VMAT y 3DCRT. Por otra parte, para los órganos a riesgo se evidenció un aumento significativo en pulmón ipsilateral V30% (p=0.0001), Corazón V5% (p=0.0000) y V25% (p=0.0005) con la técnica VMAT comparado con 3DCRT. Para IMRT se reportó una diferencia significativa por encima en el parámetro V5% en corazón (dosis bajas) con comparación a la técnica 3DCRT. La técnica conformal demostró ser la técnica que menor entrega dosis a los OAR. En conclusión, la técnica de tratamiento VMAT trae consigo una mejor conformación de la dosis prescrita en el volumen objetivo (PTV). Sin embargo, representa un incremento en las dosis entregadas a pulmón ipsilateral, corazón y mama contralateral. Con la técnica 3DCRT, se evidencia una disminución significativa de la dosis en los órganos a riesgo, no obstante, el volumen que recibe la dosis de prescripción es mayor en comparación con el volumen objetivo (PTV), es decir, las curvas de isodosis no se ajustan al PTV. Finalmente, para la técnica de Radioterapia de Intensidad Modulada (IMRT), se observa que no hay diferencia significativa conformando la dosis en el volumen objetivo en comparación con la técnica conformal, aunque si presenta diferencia significativa favorable en la dosis entregada a los órganos a riesgo en comparación con la técnica VMAT. Por lo tanto, con los datos obtenidos y evaluando el alcance del estudio, se recomienda el uso de Radioterapia Conformal 3DCRT, sin embargo, este estudio no evalúa los efectos cosméticos de cada técnica, razón que podría decantar la decisión para optar por el uso de las técnicas IMRT y VMAT, a pesar de las dosis entregadas a los órganos a riesgo. Adjuvant radiotherapy for breast cancer is a standard treatment used to improve local control tumor and overall survival. For Latin America and Caribbean, an increase in the incidence of new cases of 47% is estimated for the year 2040. [1] This research works with the acceptance dosimetric criteria of the UK-FAST-FORWARD trial (26 Gy – 5 Fx), in order to evaluate the treatment technique VMAT (Volume Modulated Intensity Arc Therapy) in comparison with the generally used techniques IMRT (Intensity Modulated Radiotherapy) and 3DCRT (3D Conformal Radiotherapy), for a random sample of 20 patients. It is important to note that these dosimetric criteria will apply to all of the techniques mentioned, including VMAT, IMRT, and 3DCRT. Data science analysis software called STATA was used to verify that the differences observed in the samples have statistical significance. This analysis showed a significant improvement (p<0.05) of the conformity index for the IMRT and VMAT techniques compared to the conformal technique, no statistically significant differences were observed for the homogeneity index between VMAT and 3DCRT. On the other hand, for the organs at risk, there is a significant increase in the ipsilateral lung V30% (p=0.0001), Heart V5% (p=0.0000) and V25% (p=0.0005) with the VMAT technique compared with 3DCRT. For IMRT, a significant difference was reported above in the V5% parameter compared to the 3DCRT technique. For IMRT, a significant difference was reported in the parameter V5% in heart (low doses) compared to the 3DCRT technique. The conformal technique proved to be the technique that delivered the lowest dose to the OARs. In conclusion, the VMAT treatment technique brings with it a better conformation of the prescribed dose into the target volume (PTV). However, it represents an increase in the doses delivered to the ipsilateral lung, heart, and contralateral breast. With the 3DCRT technique, a significant decrease in the dose in the organs at risk is evidenced, however, the volume that receives the prescription dose is greater compared to the target volume (PTV), that is, the isodose curves do not conform to the PTV. Finally, for the Intensity Modulated Radiotherapy (IMRT) technique, it is observed that there is no significant difference conforming the dose in the target volume compared to the conformal technique, although it does present a significant favorable difference in the dose delivered to the organs at risk. compared to the VMAT technique. Therefore, with the data obtained and evaluating the scope of the study, the use of 3DCRT Conformal Radiotherapy is recommended; however, this study does not evaluate the cosmetic effects of each technique, which could lead to the decision to opt for the use of IMRT and VMAT techniques, despite the doses delivered to the organs at risk. Therefore, the decision is subject to clinical and technical criteria and cannot be totally conclusive with the scope of this study.Palabras clave
