dc.description.abstract | Los adultos mayores han sido uno de los grupos demográficos más afectados con la llegada de la pandemia de COVID-19. Por lo anterior, representan un reto en la atención clínica por parte de los distintos equipos de salud. El objetivo de este estudio es determinar los factores asociados a la mortalidad intrahospitalaria en los pacientes mayores de 60 años, valorados por el servicio de Geriatría, que presentaron infección por SARS-CoV-2 y fueron hospitalizados en el Hospital Militar Central entre marzo de 2020 y marzo de 2021. Métodos: Estudio transversal con componente analítico que involucró pacientes mayores de 60 años con diagnóstico de infección SARS-CoV-2 mediante prueba específica. Se hizo un análisis descriptivo de todas las variables involucradas adicionalmente se realiza un análisis bivariado cuya variable dependiente fue la mortalidad. Por todo lo anterior, se recurre a la construcción de un modelo de regresión logística no condicional para variable dependiente. Las variables cuya asociación estuvo con la presencia de mortalidad intrahospitalaria fueron la edad mayor a 75 años (OR 2.64; p=0.028), fragilidad medida por CFS (OR 1.23, p=0.035), ventilación mecánica (OR 4.44 p<0.001) y linfopenia (OR 2.96, p<0.001). Conclusiones: La fragilidad se comportó como un factor de riesgo independiente que se asocia con mortalidad intrahospitalaria en pacientes con infección por SAR-CoV-2, por cada punto de aumento en la escala CFS existe un incremento en el riesgo de mortalidad en un 23%. Otros factores como fueron la edad, la ventilación mecánica y la linfopenia también tuvieron comportamientos como factores de riesgo independientes. (Texto tomado de la fuente). |
dc.description.abstract | Older adults have been one of the demographic groups most affected by the arrival of the COVID-19 pandemic. Therefore, they represent a challenge in clinical care by the different health teams. The objective of this study is to determine the factors associated with in- hospital mortality in patients over 60 years of age, assessed by the Geriatrics service, who presented SARS-CoV-2 infection and were hospitalized at the Central Military Hospital between March 2020 and March 2021. Methods: Cross-sectional study with an analytical component that involved patients over 60 years of age with a diagnosis of SARS-CoV-2 infection through a specific test. A descriptive analysis of all the variables involved was carried out. Additionally, a bivariate analysis was carried out, the dependent variable of which was mortality. For all of the above, the construction of a non-conditional logistic regression model for the dependent variable is used. The variables whose association was with the presence of in-hospital mortality were age over 75 years (OR 2.64; p=0.028), frailty measured by CFS (OR 1.23, p=0.035), mechanical ventilation (OR 4.44 p<0.001) and lymphopenia (OR 2.96, p<0.001). Conclusions: Frailty behaved as an independent risk factor that is associated with in-hospital mortality in patients with SAR-CoV-2 infection, for each point of increase in the CFS scale there is an increase in the risk of mortality by 23%. Other factors such as age, mechanical ventilation and lymphopenia also acted as independent risk factors. |
