Correlación de los hallazgos ecográficos pleuropulmonares con el puntaje tomográfico por Goh en pacientes con esclerosis sistémica o artritis reumatoide con enfermedad pulmonar intersticial

La enfermedad pulmonar intersticial (EPI) es una de las principales causas de morbimortalidad en pacientes con artritis reumatoide (AR) y esclerosis sistémica (ES). La tomografía de tórax es el examen de elección para el diagnóstico, tratamiento y seguimiento de los pacientes; sin embargo, el costo, radiación ionizante y disponibilidad limita su uso. La ecografía pleuropulmonar en protocolo acortado de 14 puntos es una alternativa diagnóstica que ha demostrado buena correlación con hallazgos y puntajes tomográficos en EPI; sin embargo, la correlación de los hallazgos ecográficos de líneas B y anormalidades pleurales con el puntaje tomográfico de Goh es desconocido. Objetivos: Estimar la correlación entre el número de líneas B y anomalías pleurales identificadas por ecografía pleuropulmonar con protocolo acortado de 14 puntos y el puntaje tomográfico de Goh para determinar la extensión de la enfermedad pulmonar intersticial en pacientes con AR o ES. Métodos: Estudio de pruebas diagnósticas en pacientes con diagnóstico de AR o ES y EPI que asistieron a la consulta externa del servicio de reumatología del Hospital Universitario Nacional; con realización de ecografía pleuropulmonar con protocolo acortado de 14 puntos y análisis de la correlación con índice tomográfico de Goh. Resultados Se incluyó un total de 72 pacientes, con una edad media de 62 años, 82 % mujeres, 52,8 % con esclerosis sistémica y el restante con artritis reumatoidea. Más del 50% de los pacientes tenían un compromiso menor del 5% cuantificado por tomografía. En el análisis de correlación entre la tomografía de tórax y el conteo total de líneas B o número de ventanas con alteración de la línea pleural se encontró que en imágenes ecográficas lineales era del 0,61 (Correlación de Rho de Sperman) (p<0,001) para la presencia de líneas B y de 0,63 (p<0,001) para las alteraciones pleurales y en imágenes convexas era del 0,63 (p<0,001) y del 0,64 (p<0,001) respectivamente. Se determinó el rendimiento discriminado de la ecografía pleuropulmonar en la detección de anormalidades pleuropulmonares, según el grado de severidad de la EPID definido por tomografía, encontrando que éste es mayor cuando el compromiso intersticial supera el 10% con un AUC cercano a 0,8 (IC 0,7-0,9). Conclusiones La ecografía pleuropulmonar con protocolo acortado de 14 puntos demuestra un buen grado de correlación con el índice tomográfico de Goh para la cuantificación de la EPI en pacientes con AR y ES. En especial demuestra mayor rendimiento en pacientes con un compromiso superior al 10 %, que es relevante al momento de definir tratamientos específicos. Interstitial lung disease (ILD) is one of the main causes of morbidity and mortality in patients with rheumatoid arthritis (RA) and systemic sclerosis (SS). Chest tomography is the test of choice for the diagnosis, treatment, and monitoring of patients; however, cost, ionizing radiation, and availability limit its use. Pleuropulmonary ultrasound in a shortened 14-point protocol is a diagnostic alternative that has demonstrated good correlation with tomographic findings and scores in PID; However, the correlation of ultrasound findings of B lines and pleural abnormalities with the Goh tomographic score is unknown. Objectives: Estimate the correlation between the number of B lines and pleural abnormalities identified by pleuropulmonary ultrasound with a shortened 14-point protocol and the Goh tomographic score to determine the extent of interstitial lung disease in patients with RA or SS. Methods: Study of diagnostic tests in patients diagnosed with RA or SS and ILD who attended the outpatient clinic of the rheumatology service of the National University Hospital; with pleuropulmonary ultrasound performed with a shortened 14-point protocol and analysis of the correlation with the Goh tomographic index. Results A total of 72 patients were included, with a mean age of 62 years, 82% women, 52.8% with systemic sclerosis and the remainder with rheumatoid arthritis. More than 50% of patients had less than 5% involvement quantified by tomography. In the correlation analysis between the chest tomography and the total count of B lines or number of windows with alteration of the pleural line, it was found that in linear ultrasound images it was 0.61 (Sperman's Rho Correlation) (p<0.001 ) for the presence of B lines and 0.63 (p<0.001) for pleural alterations and in convex images it was 0.63 (p<0.001) and 0.64 (p<0.001) respectively. The discriminative performance of pleuropulmonary ultrasound in the detection of pleuropulmonary abnormalities was determined, according to the degree of severity of ILD defined by tomography, finding that this is greater when interstitial involvement exceeds 10% with an AUC close to 0.8 (CI 0.7-0.9). Conclusions Pleuropulmonary ultrasound with a shortened 14-point protocol demonstrates a good degree of correlation with the Goh tomographic index for the quantification of ILD in patients with RA and SS. It demonstrates greater performance in patients with a compromise greater than 10%, which is relevant when defining specific treatments.
