Factores determinantes para la implementación del método madre canguro: una revisión de alcance

Abstract

Introducción: El Método Madre Canguro ha demostrado ser útil y costo-efectivo en el cuidado, tratamiento y recuperación de los recién nacidos pretérmino y/o de bajo peso al nacer, pese a esto aún existen lugares del mundo en los cuales no se logra implementar por diversas circunstancias, de allí surge la necesidad de explorar la evidencia sobre dónde se ha implementado, cuáles son las barreras y facilitadores determinantes durante dicho proceso. Objetivo: indagar acerca del alcance y tipo de evidencia existente en relación con los factores determinantes y el nivel de implementación del Método Madre Canguro en el mundo. Metodología: Estudio de revisión de alcance que siguió la metodología propuesta por el Instituto Johana Briggs JBI de la Universidad de Adelaida. Criterios de Inclusión: Se incluyeron estudios en idiomas, español, inglés y portugués, publicados desde1978 hasta el 2022 teniendo en cuenta el año del surgimiento del Método Madre Canguro. Método: Se utilizó una metodología de búsqueda en tres pasos, en primer lugar, una búsqueda libre de la temática, en MEDLINE (PubMed) y CINALH (EBSCO), luego, búsqueda exhaustiva y sistemática en las siguientes bases de datos: SCIELO, SCOPUS, PUMED, CINALH, EBSCO, MEDLINE, usando la siguientes ecuaciones[(Metodo Madre-Canguro) OR (Kangaroo-Mother Care Method) AND (Barreras) OR (Barriers)]; [(Metodo Madre-Canguro) AND (Facilitadores OR Potencializadores)]; [(Metodo Madre-Canguro) AND [(Implementación) OR Implementation)]. Posterior a esto, se realizó una búsqueda secundaria, a partir de las fuentes seleccionadas en texto completo. Una vez, finalizada la búsqueda, los dos investigadores (Estudiante de maestría y directora de tesis) realizaron la selección de los estudios, los desacuerdos fueron solucionados por medio de consenso entre ellos. Se realizó la extracción de los datos y se empleó análisis temático. Resultados: la búsqueda arrojó un total de 197 artículos que luego de ser revisados por pertinencia y de acuerdo con los criterios de inclusión, fueron analizados a profundidad 43 artículos en texto completo. La implementación del método madre canguro en el mundo es variable, los factores determinantes incluidos como barreras y facilitadores más relevantes estuvieron relacionados con el nivel gubernamental, la infraestructura, el acceso y el talento humano en salud. Dentro de los facilitadores se encuentra que el liderazgo en todos los niveles es indispensable, seguido de la educación, la capacitación y la motivación acerca del Método Madre canguro (MMC) al personal, los padres y directivos, quienes, al conocer los beneficios de dicha estrategia, mejoran la implementación del MMC en diferentes partes del mundo. Conclusión: los factores determinantes en la implementación del método madre canguro pueden convertirse en barreras o facilitadores acorde a como sean manejados y gestionados. La generación de políticas a nivel nacional, local e institucional, así como el apoyo de la gerencia en la disposición de recursos, son determinantes para la implementación y beneficios del método. No existen datos significativos que aporten a valorar el nivel de implementación del MMC a nivel mundial, es llamativa la ausencia de América y la alta participación de África y Asia (Texto tomado de la fuente).

Introduction: The Kangaroo Mother Care method has proven to be useful and cost-effective in the care, treatment and recovery of preterm and/or low birth weight newborns, despite this there are still places where it is not possible to implement it. There arises the need to explore the evidence on where it has been implemented, what are the determining barriers and facilitators in the implementation Objective: to inquire about the scope and type of existing evidence in relation to the determining factors in the implementation of the kangaroo mother method in neonatal care units. Methodology: Scoping review study that followed the methodology proposed by the Johana Briggs JBI Institute at the University of Adelaide. Inclusion Criteria: Studies in languages, Spanish, English and Portuguese, published from 1978 to 2022, taking into account the year of the appearance of the Kangaroo Mother Care Method, were included. Method: A three-step search methodology was used, first, a free search of the subject, in MEDLINE (PubMed) and CINALH (EBSCO), then, an exhaustive and systematic search in the following data bases: SCIELO, SCOPUS, PUMED, CINALH, EBSCO, MEDLINE, using the following equations [(METODO MADRE-KANGAROO) OR (KANGAROO-MOTHER CARE METHOD) AND (BARRERAS) OR (BARRIERS)]; [(MOTHER-KANGAROO METHOD)AND (FACILITATORS OR POTENTIALIZERS)]; [(KANGAROO MOTHER METHOD) AND [(IMPLEMENTACIÓN) OR IMPLEMENTATION)]. After this, a secondary search was carried out, based on the selected sources in full text. Once the search was completed, the two researchers (Master's student and thesis supervisor) made the selection of the studies, disagreements were resolved by consensus between them. Data extraction and the mastic analysis were performed. Results: the search yielded a total of 197 articles that, after being reviewed for relevance and according to the inclusion criteria, 43 full-text articles were analyzed in depth. The implementation of the kangaroo mother method in the world is variable, the determining factors included as the most relevant barriers and facilitators were related to the government al level, infrastructure, access and human talent in health. Among the facilitators, it is found that leadership at all level sis essential, followed by education, training and motivation about the Kangaroo Mother Care Method (KMC) for staff, parents and managers, who, knowing the benefits of such strategy, improve the implementation of the KMC in different part soft the world. Conclusion: the determining factors in the implementation of the kangaroo mother method can become barriers or facilitators according to how they are handled and managed. The generation of policies at the national, local and institution al levels, as well as the support of management in the provision of resources, are decisive for the implementation of the method. There are no significant data that contribute to assessing the level of implementation of the MMC worldwide, the absence of America y the high participation of Africa and Asia are striking.
