dc.description.abstractLos sistemas de información en salud (SIS) han evolucionado bajo lógicas administrativas y financieras fragmentadas que limitan su capacidad para apoyar la toma de decisiones clínicas y representar el carácter holístico y longitudinal del proceso salud-enfermedad de los individuos. Esta fragmentación restringe la disponibilidad de información, afecta la continuidad del cuidado y mantiene un rol pasivo de las personas frente al gobierno de sus propios datos en salud. Como respuesta a estas limitaciones, este trabajo propone una arquitectura de referencia para sistemas de información en salud basada en principios de gobierno de datos y centrada en un objeto digital portable controlado por el individuo, que integra los determinantes biológicos, conductuales, ambientales y de servicios de salud del proceso salud-enfermedad. La metodología empleada corresponde a ProSA-RA, desarrollada en cuatro fases: (i) revisión sistemática de literatura para identificar arquitecturas, patrones y dominios relevantes del campo; (ii) definición conceptual y estructural de los dominios del proceso salud-enfermedad; (iii) diseño de una arquitectura de información, construcción de un objeto digital portable codificado en esquemas Avro y síntesis de una arquitectura de referencia basada en principios de Ecosistemas de Software y Sistema-de-Sistemas; y (iv) evaluación mediante escenarios, casos de estudio y una prueba de concepto. La arquitectura propuesta mejora la disponibilidad y trazabilidad de la información, fortalece la toma de decisiones basadas en evidencia y devuelve a los individuos la gobernabilidad de sus datos. Asimismo, facilita la integración de sistemas heterogéneos, soporta análisis longitudinales y habilita capacidades para analítica avanzada. dc.description.abstractHealth information systems (HIS) have evolved through fragmented administrative and financial logics that limit their capacity to support clinical decision-making and to reflect the holistic and longitudinal nature of the health–disease process of individuals. This fragmentation restricts information availability, reduces continuity of care, and reinforces the passive role of individuals in the management of their own health data. Motivated by these limitations, this research proposes a reference architecture for health information systems grounded in data governance principles and centered on an individual-controlled portable digital object that integrates the biological, behavioral, environmental, and healthcare-service determinants of the health–disease process. The study follows the ProSA-RA methodology, structured into four phases: (i) a systematic literature review to identify architectural approaches, patterns, and domains relevant to health information systems; (ii) definition of the domains and conceptual foundations of the health–disease process; (iii) design of an information architecture, development of a portable digital object encoded in Avro schemas, and synthesis of a reference architecture based on Software Ecosystems and System-of-Systems principles; and (iv) evaluation through scenarios, case studies, and a proof of concept. The proposed architecture improves information availability, supports evidence-based decision-making, and strengthens the governance capability of individuals over their own health data. It also enables integration across heterogeneous systems, supports longitudinal analysis, and provides mechanisms for advanced analytics.
