dc.description.abstractGeneral anaesthesia is an essential anaesthetic technique used in most of the patients taken to surgery. Although the cost of the anaesthesia is a lower percentage compared with the total cost of the surgical patient’s service, the great quantity of procedures carried out represents an important total cost for any healthcare system. The objective of this research is to quantify and compare the direct costs of the balanced anaesthetic techniques with isoflurane, sevoflurane and desflurane, and the intravenous general anaesthetic in Colombia. An economic cost reduction analysis was carried out, based on an economic model made with the determinant variants of the differential costs for the general anaesthetic techniques, using values adjusted to the Colombian context. 10.000 possible scenarios were simulated using the Monte Carlo method and it was found that, on average, balanced general anaesthesia with desflurane was the most expensive of the techniques, total intravenous anaesthesia was cheaper only compared to balanced general anaesthesia with desflurane, and the cheapest anaesthetic technique was isoflurane, followed by sevoflurane and TIVA. The main difference between the techniques used for the administration of general anaesthesia in a simulation of adult patients taken to a non-cardiac surgery was their cost. Therefore, rational decision making should take into account such difference, permitting a better cost-opportunity relation for the use of scarce resources, available for healthcare attention.spa
dc.description.abstractLa anestesia general es una técnica anestésica esencial para la atención de la mayoría de los pacientes llevados a cirugía. Aunque el costo de la anestesia representa un porcentaje menor con respecto al costo total de la atención del paciente quirúrgico, el gran número de procedimientos realizados representa un costo total importante para cualquier sistema de salud. Esta investigación tiene por objetivo cuantificar y comparar los costos directos de las técnicas de anestesia general balanceada con isoflurano, sevoflurano y desflurano, y la anestesia general intravenosa en Colombia. Se hizo un análisis económico tipo minimización de costos, basado en un modelo económico construido con las variables determinantes de los costos diferenciales para las técnicas de anestesia general, utilizando valores que se ajusten al contexto colombiano. Se simularon 10.000 escenarios posibles utilizando el método estadístico de Montecarlo y se encontró que en promedio la anestesia general balanceada con desflurano fue la más costosa de todas las técnicas, la anestesia total intravenosa solo fue más barata que la anestesia general balanceada con desflurano y la técnica anestésica más barata fue el isoflurano, seguida por el sevoflurano y la TIVA. La principal diferencia entre las técnicas utilizadas para la administración de anestesia general en una simulación de pacientes adultos llevados a cirugía no cardiaca fue el costo de las mismas. Por lo anterior, una toma de decisiones racional debería tener en cuenta dicha diferencia, permitiendo una mejor relación costo oportunidad en el uso de los recursos escasos, disponibles para la atención en salud.spa
