dc.description.abstract | La predicción de la respuesta a volumen en pacientes adultos bajo anestesia general con monitorización básica y sin posibilidad de toma intraoperatoria de índices ecocardiográficos, es limitada. El objetivo del presente estudio fue Determinar la relación entre el índice de perfusión y la variabilidad de la presión de pulso inducida por el ciclo respiratorio en pacientes adultos bajo anestesia general con el fin de evaluar de forma indirecta la capacidad del índice de perfusión para predecir la respuesta a volumen. Se planteo un estudio observacional de corte transversal analítico que incluyó 40 pacientes adultos entre 18 y 70 años llevados a anestesia general y ventilación mecánica invasiva controlada con monitorización de la variabilidad de la presión de pulso e índice de perfusión con monitores Mindray BeneView T5 e iMEC12 en el Hospital Universitario Nacional de Colombia entre Marzo y Octubre de 2020. La muestra estuvo conformada por 40 individuos, con predominio del sexo masculino (51,2%) con una media en la edad de 55.11±15.54 años, 64.5±11.04 kg para el peso, y 163.15±8.53 cm para la talla. En la totalidad de los pacientes, el dispositivo de vía aérea usado fue el tubo orotraqueal. La ventilación mecánica fue controlada con parámetros protectores. Se encontró moderada correlación inversa entre el IP y la VPP tras presentarse sangrado estimado en 300ml, 500ml, 1000ml y tras bolo de cristaloides de 500ml sin diferencia estadística significativa. Se concluye que el índice de perfusión como variable estática podría predecir el valor de variabilidad de la presión de pulso, se requiere ampliar el tamaño muestral para establecer la significancia estadística de dicha relación. | spa |
dc.description.abstract | The prediction of the volume response in adult patients under general anesthesia with basic monitoring and without the possibility of intraoperative measurement of echocardiographic index is limited. The objective of the present study was to determine the relationship between the perfusion index and the variability of the pulse pressure induced by the respiratory cycle in adult patients under general anesthesia in order to indirectly evaluate the ability of the perfusion index to predict the response to volume. An analytical cross-sectional observational study was proposed that included 40 adult patients between 18 and 70 years old undergoing general anesthesia and controlled invasive mechanical ventilation with monitoring of pulse pressure variability and perfusion index with Mindray BeneView T5 and iMEC12 monitors in the National Hospital of Colombia between March and October of 2020. The sample consisted of 40 individuals, predominantly male (51.2%) with a mean age of 55.11 ± 15.54 years, 64.5 ± 11.04 kg for weight, and 163.15 ± 8.53 cm for height. In all the patients, the airway device used was the orotracheal tube. Mechanical ventilation was controlled with protective parameters. A moderate inverse correlation was found between PI and PPV after bleeding estimated at 300ml, 500ml, 1000ml and after bolus of 500ml crystalloids without statistically significant difference. It is concluded that the perfusion index as a static variable that could predict the pulse pressure variation value, it is necessary to expand the sample size to establish the statistical significance of this relationship. | spa |
