| dc.description.abstract | Introducción: La anemia preoperatoria es una condición prevalente que incrementa la morbimortalidad quirúrgica y la necesidad de transfusión. El Patient Blood Management (PBM) se propone como una estrategia efectiva para optimizar la sangre del paciente sin embargo, su adopción depende de la percepción de profesionales y de las instituciones. Objetivo: evaluar la percepción de los especialistas en anestesiología sobre los facilitadores y barreras para la implementación de estrategias de “Patient Blood Management” perioperatorias en pacientes adultos llevados a cirugía en Bogotá, Colombia en 2025. Métodos: Se realizó un estudio transversal mediante una encuesta estructurada aplicada a anestesiólogos de Bogotá difundida de manera virtual. La encuesta incluyó 29 preguntas con respuestas en forma de escala de Likert (1-5) sobre detección de anemia, suplementación de hierro y ácido fólico, uso de protocolos institucionales y disposición hacia la implementación de PBM. Los datos fueron analizados de manera descriptiva y se identificaron barreras y facilitadores percibidos. Resultados: Los anestesiólogos mostraron alta receptividad frente a los cambios propuestos por las estrategias de PBM. Entre los facilitadores, se destacó la existencia de protocolos institucionales y la práctica consolidada de valoración preanestésica con al menos siete días de antelación, considerada una oportunidad para la detección y corrección de la anemia. Como barreras, se identificaron la falta de estandarización en la suplementación de hierro, la limitada disponibilidad de hierro intravenoso y la ausencia de políticas estandarizadas de PBM en las instituciones. Conclusión: La percepción favorable de los anestesiólogos y la existencia de estructuras parcialmente consolidadas constituyen oportunidades importantes para la implementación de programas piloto de PBM en el contexto colombiano. No obstante, aún persisten limitaciones relacionadas con la disponibilidad de recursos y la necesidad de fortalecer el liderazgo en las instituciones, las cuales deben ser abordadas para garantizar una adopción factible. Patient Blood Management (PBM) has been proposed as an effective strategy to optimize the patient`s own blood, however, its adoption depends on the perception of healthcare professionals and institutional support. Objective: To assess the perception of anesthesiology specialists regarding the facilitators and barriers for the implementation of perioperative Patient Blood Management (PBM) strategies in adult patients undergoing surgery in Bogotá, Colombia, in 2025. Methods: A cross-sectional study was conducted through a structured online survey distributed to anesthesiologists in Bogotá. The survey included 29 items with Likert-scale responses (1-5) covering anemia detection, iron and folic acid supplementation, use of institutional protocols, and willingness to implement PBM. Data were analyzed descriptively, and perceived barriers and facilitators were identified. Results: Anesthesiologists showed high receptivity to the changes proposed by PBM strategies. Among the facilitators, the presence of institutional protocols and the established practice of pre- anesthetic evaluation at least seven days before surgery were highlighted as opportunities for anemia detection and correction. Barriers included the lack of standardization in iron supplementation, limited availability of intravenous iron, and the absence of standardized PBM policies within institutions. Conclusion: The favorable perception of anesthesiologists and the existence of partially established structures represent important opportunities for the implementation of pilot PBM programs in the Colombian context. However, limitations related to resource availability and the need to strengthen institutional leadership must be addressed to ensure feasible adoption. | eng |
