Influencia de las prácticas de gestión de la cadena de abastecimiento en el desempeño sostenible hospitalario: Un estudio empírico en el contexto colombiano

dc.description.abstractDesde la literatura y la práctica se evidencia un interés creciente en las cadenas de abastecimiento hospitalarias, teniendo en cuenta el comportamiento ascendente del gasto en salud a nivel mundial, la alta participación de los costos logísticos en los presupuestos hospitalarios y la contribución potencial de la gestión de la cadena de abastecimiento en la disminución de tales costos. Mas aún, aspectos ambientales y sociales de las cadenas de abastecimiento hospitalarias han venido suscitando cada vez más preocupación a nivel mundial, ya que los hospitales son intensivos en el consumo de recursos ambientales, generación de residuos y tienen impactos significativos en sus trabajadores y en las comunidades circundantes. Aunque la literatura en el campo de la gestión de cadenas de abastecimiento sostenibles es cada vez más profusa en el sector manufacturero, es escasa en servicios de salud y no parece existir claridad sobre cómo los hospitales pueden ser más sostenibles ambiental, social y económicamente a través de la gestión de sus cadenas de abastecimiento. En consecuencia, esta tesis doctoral tiene como objetivo analizar, desde un punto de vista empírico, la influencia de un conjunto de prácticas de gestión de la cadena de abastecimiento en el desempeño sostenible en instituciones prestadoras de servicios de salud en Colombia. A partir de la literatura, se definieron tres constructos ambientales: gestión ambiental interna, compras verdes y operaciones verdes; y cuatro constructos sociales: condiciones de trabajo, gestión de la equidad, compras sociales y desarrollo y participación de la comunidad. Así mismo, el tamaño, el tipo de hospital y la complejidad de los servicios prestados fueron considerados como variables moderadoras. Se diseñó, validó y aplicó una encuesta en una muestra de 210 instituciones prestadoras de servicios de salud de 27 departamentos de Colombia. La información recolectada se analizó mediante el modelamiento de ecuaciones estructurales. Los resultados muestran que las operaciones verdes tienen un efecto positivo significativo en el desempeño ambiental, de la misma manera que la gestión de la equidad y las compras sociales lo tienen en el desempeño social. A su vez, el estudio aportó evidencia empírica sobre la influencia del desempeño social en el económico, si bien no se confirmó un impacto significativo del desempeño ambiental en el social y en el económico. En cuanto a las variables moderadoras, un análisis multigrupo reveló diferencias estadísticamente no significativas. Sin embargo, la aplicación de pruebas estadísticas no paramétricas indica que las instituciones prestadoras de servicios más grandes, privadas y que prestan servicios de alta complejidad, presentan mayores niveles de implementación de prácticas de gestión de la cadena de abastecimiento ambientales y sociales. Este trabajo contribuye al entendimiento de cuáles prácticas de gestión pueden ser decisivas para el logro de cadenas de abastecimiento hospitalarias más sostenibles desde las perspectivas ambiental, social y económica, en el contexto de un país emergente. Los resultados constituyen una contribución novedosa tanto teórica como práctica, teniendo en cuenta que permiten acumular evidencia en las relaciones estudiadas, estimular la generación de nuevas variables, operacionalizaciones de constructos y modelos de investigación. Adicionalmente, los resultados proveen una base estructurada y confirmada para los hospitales sobre las diversas formas que pueden tomar las prácticas de gestión de la cadena de abastecimiento sostenible, las métricas de desempeño sostenible y, hasta cierto grado, los efectos esperados de su implementación. dc.description.abstractThere is evidence of growing interest in hospital supply chains in the literature and in practice due to the increase of healthcare expenditure, the representativeness of logistics costs in hospital budgets, and the possible contribution of supply chain management to organize logistics processes more effectively and efficiently. Furthermore, aspects that include the environmental and social impact of hospital supply chains have been raising global concern, as hospitals are intensive in environmental resource consumption, waste generation, and have significant impacts on their workers and their surrounding communities. Although the literature in the field of sustainable supply chain management is increasingly profuse in the manufacturing sector, it is scarce in health services and there does not seem to be clarity on how hospitals can be more environmentally, socially, and economically sustainable through the management of their supply chains. Consequently, this doctoral thesis aims to empirically analyze the influence of a set of sustainable supply chain management practices on sustainable performance of Colombian healthcare providers. Based on the literature, three environmental constructs were defined: internal environmental management, green purchasing, and green operations; and four social constructs: working conditions, equity management, social purchasing, and community development and participation. Moreover, hospital size, hospital type and service complexity were considered as moderating variables. A structured survey was designed, validated, and applied to 210 hospitals from 27 states in Colombia. The information collected was analyzed using structural equation modeling. Results show that green operations have a positive effect on environmental performance, as well as equity management and social purchasing on social performance. Empirical evidence also shows the influence of social performance on the economic performance, despite significant effects of the environmental performance on the social and the economic performance were not confirmed. Regarding the moderating variables, a multigroup analysis revealed no significant differences. However, the application of non-parametric statistical tests indicates that the largest, private hospitals and those which provide highly complex services seem to have better levels of implementation of environmental and social supply chain management practices. This research contributes to the understanding of which management practices may be decisive for the achievement of more sustainable hospital supply chains from the environmental, the social and the economic dimensions, in the context of an emerging country. The results constitute a novel contribution, both theoretical and practical, as they may be added to the accumulation of evidence on the relationships studied and may stimulate the generation of new variables, construct operationalizations and research models. In addition, results provide a structured and confirmed basis for hospitals on the various forms that sustainable supply chain management practices and performance metrics can take and, to some degree, the expected effects of their implementation.eng
