Evaluación radiobiológica y dosimétrica de las planeaciones de Cáncer de seno izquierdo con refuerzo escalado y simultáneo

dc.description.abstractThe greatest therapeutic benefit of radiotherapy is achieved by depositing the highest dose in the treated tissue by keeping the nearby organs’ doses below established limits. The Clinac iX, clinical linear accelerator manufactured by Varian Inc., present at the Department of Medical Physics of the Instituto de Cáncer Carlos Ardila Lülle in the University Hospital of the Fundacion Santa Fe de Bogotá allows the application of three different techniques for external radiotherapy treatments, being the conformational technique the routinely one used for the hypofractionated treatment in breast volume (4005 cGy / 267 cGy in 15 sessions) with sequential scaled boost in the bed surgical area (5 additional sessions of 200 cGy). With the intention of using the modulation techniques and applying the integrated simultaneous boost (synchronously in 15 sessions deposit 4005 cGy / 267 cGy in breas volume and 4500 cGy / 300 cGy within the boost volume ) this evaluation was carried out using the Gay model and Niemierko to determine theTumor Control Probability and the Normal Tissue Complication Probability from the calculation of the Equivalent Uniform Dose made with the dose distributions determined by the EclipseTMtreatment planning system of Varian Inc.. With the endorsement of the Ethics Committee and the Subdirection of Clinical Studies of the University Hospital, images of tomographic simulations (8 patients referred to the service) were used to project 5 treatment schedules that meet the homogeneity criteria (HI < 0.1) and conformation (CI > 0.9) of the center, being the arcotherapy with simultaneous boost slightly the best dose conformation (2%). The comparison between the di erent techniques showed that the reduction in total treatment time slightly reduces the tumor control in the breast, but keeps it in the boost zone when the integrated boost is applied with the modulated intensity techniques; although arcotherapy irradiations increase the dose received by the contralateral organs, the clinical e ects will not be significant in the integrated scheme but in the sequential one, where the increase, from 5% to 10%, of the probability of having symptomatic pneumonitis.spa
dc.description.abstractEl mayor beneficio terapéutico de la radioterapia se consigue depositando la mayor dosis en el tejido a tratar manteniendo las dosis de los órganos cercanos por debajo de los límites establecidos. El acelerador lineal clínico, Clinac iX fabricado por Varian Inc., con que cuenta el departamento de Física Médica del Instituto de Cáncer Carlos Ardila Lülle del Hospital Universitario de la Fundación Santa Fe de Bogotá permite aplicar tratamientos de radioterapia externa con tres diferentes técnicas, siendo la técnica conformacional la empleada rutinariamente para el tratamiento hipofraccionado en el volumen del seno (4005 cGy/267 cGy en 15 sesiones) con refuerzo escalado secuencial en la zona del lecho quirúrgico (5 sesiones adicionales de 200 cGy). Con la intención de utilizar las técnicas de modulación y de aplicar el refuerzo simultáneo integrado (en 15 sesiones depositar 4005 cGy/267 cGy en el seno y sincrónicamente 4500 cGy/300 cGy en el volumen de refuerzo) se realizó esta evaluación usando el modelo de Gay y Niemierko para determinar la Probabilidad de Control Tumoral y la Probabilidad de Complicaciones en Tejidos Sanos a partir, del cálculo de la Dosis Uniforme Equivalente, obtenido de las distribuciones de dosis determinadas por el sistema de planeación de tratamientos EclipseTMde Varian Inc. Con el aval del Comité de Ética y de la Subdirección de Estudios Clínicos del Hospital Universitario se emplearon las imágenes de las tomografías de simulación (8 pacientes remitidas al servicio) para proyectar 5 planificaciones de tratamiento que se ajusten a los criterios de homogeneidad (HI < 0.1) y conformación (CI > 0.9) del centro, siendo la arcoterapia con refuerzo simultáneo ligeramente la de mejor conformación de la dosis (2%) . La comparación entre las diferentes técnicas mostró que la reducción en el tiempo total de tratamiento reduce ligeramente el control tumoral en la mama, pero lo mantiene en la zona de refuerzo cuando se aplica el refuerzo integrado con las técnicas de intensidad modulada; aunque las irradiaciones con arcoterapia aumentan la dosis recibida por los órganos contralaterales, los efectos clínicos no serán significativos en el esquema integrado pero si en el secuencial, donde se evidencia el incremento, del 5% al 10%, de la probabilidad de presentar neumonítis sintomática.spa
