Calidad de vida, estado de ánimo y salud sexual en personas adultas portadoras de CDI

Abstract

Introducción: El desfibrilador automático implantable (ICD por sus siglas en inglés) detecta arritmias graves con el fin de tratarlas y recuperar el ritmo cardiaco normal. Estudios previos revelan que usar un ICD genera cambios en la calidad de vida, el estado de ánimo y la salud sexual de quienes lo portan. La poca evidencia disponible impide conocer con precisión cómo se relacionan estas variables. Objetivo: Establecer cómo se relacionan el estado de ánimo y la salud sexual con la calidad de vida relacionada con la salud (CVRS) de personas adultas portadoras de un ICD, atendidas en el Servicio de Electrofisiología de la Clínica Mayo, en Jacksonville, Florida, Estados Unidos, durante los primeros tres años posimplante. Método: Estudio cuantitativo, correlacional y de corte transversal en el que participaron 30 personas adultas con un ICD, atendidas en la Clínica Mayo durante los primeros tres años posimplante. Se aplicaron la encuesta Short Form Health Survey (SF-36), la escala Profile of Mood States (POMS) y el cuestionario Multidimensional Sexuality Questionnaire (MSQ). Resultados: La muestra estuvo constituida en su mayoría por personas de género masculino, con un rango de edad de entre 27 y 83 años. La CVRS mostró correlación inversa con el estado de ánimo (rho = -0,645; p < 0,001) y directa con la salud sexual (rho = 0,535; p = 0,005). No fue posible establecer una relación estadística entre la salud sexual y el estado de ánimo (rho = - 0,279; p = 0,168). A través del modelo de regresión ordinal, se confirmó que hay relación estadísticamente significativa entre la CVRS con el estado de ánimo (p = 0,014) y la salud sexual (p = 0,002). Conclusión: Se comprobó que el estado de ánimo y la salud sexual se comportan como factores que se relacionan con la CVRS, por lo que es importante identificarlos durante la valoración de enfermería para mejorar la calidad de vida de pacientes que portan un ICD. Introduction: An implantable cardioverter defibrillator (ICD) detects and treats serious arrhythmias to regain normal heart rhythm. Previous studies reveal that using an ICD produce changes in the quality of life, mood, and sexual health of people who have an ICD. The scant available evidence does not allow us to know precisely how these variables are related. Objective: To establish how mood and sexual health are related to the health-related quality of life (HRQOL) of adults having an ICD implanted between 1 and 36 months, treated at the Electrophysiology Service of the Mayo Clinic, in Jacksonville, FL, United States. Method: A quantitative, correlational, cross-sectional study, with the participation of 30 adults with an ICD, seen at the Mayo Clinic, during the first three years post implantation. The Short Form Health Survey (SF-36), the Profile of Mood States (POMS), and the Multidimensional Sexuality Questionnaire (MSQ) were used. Results: The sample consisted mostly of males, with an age range between 27 and 83 years. The HRQOL showed negative correlation with mood (rho = -0.645; p < 0.001), and positive correlation with sexual health (rho = 0.535; p = 0.005). It was not possible to identify a statistical relationship between sexual health and mood (rho = -0.279; p = 0.168). Through the ordinal regression model, it is confirmed that there is a statistically significant relationship between HRQOL and mood (p = 0.014) and sexual health (p = 0.002). Conclusion: It was found that mood and sexual health behave as factors that affect the HRQOL; therefore, identifying these factors during the nursing assessment is important to improve the quality of life of patients who have an ICD.
