Perspectivas de psiquiatras y residentes sobre la formación en psicoterapia durante la residencia de Psiquiatría en Bogotá, Colombia : una aproximación desde la teoría fundamentada

| dc.description.abstract | La formación en psicoterapia durante la residencia de psiquiatría es motivo de debate global, ante la falta de consenso sobre la pertenencia y el rol específico de la psicoterapia en el quehacer del psiquiatra. La investigación existente se ha centrado en la evaluación curricular, prestando escasa atención a las experiencias subjetivas de los residentes, llevando a una comprensión parcial del fenómeno. En este trabajo se desarrolló una explicación teórica sobre cómo los residentes y egresados de los Departamentos de Psiquiatría de Bogotá construyen sus opiniones y creencias sobre dicha formación. Se trata de una investigación cualitativa, de tipo teoría fundamentada constructivista, sustentada en el interaccionismo simbólico y en teorías sociológicas de la identidad profesional. Se entrevistaron catorce participantes de cinco universidades, realizando la recolección y el análisis concurrente de los datos siguiendo el procedimiento propuesto por Charmaz. La teoría emergente, “Construir la identidad profesional navegando entre los discursos institucionales sobre la psicoterapia”, postula que el residente configura su perspectiva sobre esta formación atribuyendo límites a la disciplina (psiquiatría), la herramienta (psicoterapia) y el sujeto (rol profesional), en función de las experiencias vividas. Este es un fenómeno inserto en el macroproceso de la conformación de la identidad profesional, donde el futuro residente crea un imaginario sobre el psiquiatra que aspira a ser y, posteriormente, al entrar a la residencia, vive la tensión constante entre identificarse con su universidad e individuarse. El tránsito hacia la individuación es impulsado por crisis en el sistema de creencias, que llevan al residente a redefinir los límites descritos -y su permeabilidad- y, por ende, a reconfigurar su perspectiva sobre el rol de la psicoterapia en el quehacer del psiquiatra. Este proceso ocurre en un contexto de socialización con la comunidad de psiquiatras, donde la psicoterapia actúa como capital simbólico en la definición de la identidad de la especialidad. Se concluye que desarrollar y reconfigurar un punto de vista sobre la formación en psicoterapia durante la residencia: 1) surge de negociar los límites del sistema de creencias propio, 2) es un elemento central en la conformación de la identidad profesional y 3) lejos de tratarse de un ejercicio exclusivamente reflexivo individual, tiene un fuerte carácter pragmático e interpersonal. | dc.description.abstract | Psychotherapy training during psychiatry residency is a subject of global debate, given the lack of consensus regarding the place and specific role of psychotherapy within the psychiatrist’s practice. Existing research has focused primarily on curricular evaluation, paying little attention to the subjective experiences of residents, which has led to a partial understanding of the phenomenon. This study developed a theoretical explanation of how residents and graduates from Psychiatry departments in Bogotá construct their opinions and beliefs regarding such training. This is a qualitative study employing constructivist grounded theory, informed by symbolic interactionism and sociological theories of professional identity. Fourteen participants from five universities were interviewed, with concurrent data collection and analysis following the procedure proposed by Charmaz. The emerging theory, “Constructing professional identity by navigating through institutional discourses on psychotherapy,” posits that residents develop their perspective on this training by attributing boundaries to the discipline (psychiatry), the tool (psychotherapy), and the subject (professional role), based on lived experiences. This phenomenon is embedded in the macro-process of Professional Identity Formation, where the future resident creates an imaginary of the psychiatrist they aspire to be and subsequently, upon entering residency, experiences a constant tension between identifying with their university and undergoing individuation. The transition toward individuation is driven by crises in the belief system, which leads the residents to redefine the boundaries—and their permeability—thereby reconfiguring their perspective on the role of psychotherapy in the psychiatrist’s practice. This process occurs within a context of socialization with the psychiatric community, where psychotherapy acts as symbolic capital in defining the identity of the specialty.
