dc.description.abstractContexto y objetivo: La ferropenia es la causa principal de anemia en la población pediátrica a nivel mundial. El tratamiento oral es el de elección, sin embargo, en ocasiones el hierro parenteral surge como una opción válida. El uso más conocido del hierro parenteral es cuando la terapia oral fracasa, pero existen otros escenarios para el uso del hierro parenteral. El objetivo de esta revisión es recopilar las indicaciones actuales disponibles en la literatura médica para la terapia con hierro parenteral en los pacientes pediátricos. Métodos: Se realizó una búsqueda de publicaciones entre 2019 y 2024, en inglés o en español, que evaluaran o recomendaran el uso de hierro parenteral en la población pediátrica, y al menos uno de los siguientes desenlaces: resolución de la anemia, aumento de los niveles de hemoglobina o beneficio del hierro parenteral en comparación con el hierro oral. Resultados: Se incluyeron un total de 21 artículos. A parte del fracaso con la terapia de hierro oral, el hierro parenteral está indicado especialmente en patologías gastrointestinales, inflamación activa, nutrición parenteral, trasplante renal, menorragia o anemia ferropénica perioperatoria. La carboximaltosa férrica y hierro sacarosa muestran un mejor perfil de seguridad comparadas con hierro dextrano. Entre 6 a 12 semanas después de su uso se observan incrementos de hemoglobina entre 1–2 g/dL. Conclusiones: El hierro parenteral tiene un gran campo de acción en la población pediátrica, es seguro y eficaz en anemia ferropénica, pero requiere monitorización y ensayos clínicos comparativos adicionales futuros. dc.description.abstractBackground and objective: Iron deficiency is the leading cause of anemia in the pediatric population worldwide Oral treatment is the option of choice; however, parenteral iron is sometimes a valid option. The most well-known use of parenteral iron is when oral therapy fails, but there are other scenarios for the use of parenteral iron. The objective of this review is to compile the current indications available in the medical literature for parenteral iron therapy in pediatric patients. Methods: A search was conducted for articles published between 2019 and 2024, in English or Spanish, that evaluate or recommend the use of parenteral iron in the pediatric population. These articles also evaluated at least one of the following outcomes: resolution of anemia, increased hemoglobin levels, or the benefit of parenteral iron compared to oral iron. Results: A total of 21 articles were included. Apart from failure with oral iron therapy, parenteral iron is especially indicated in gastrointestinal pathologies, active inflammation, parenteral nutrition, kidney transplantation, menorrhagia, or perioperative iron deficiency anemia. Ferric carboxymaltose and iron sucrose show a better safety profile compared to iron dextran. Increases in hemoglobin of 1–2 g/dL are observed between 6 and 12 weeks after use. Conclusions: Parenteral iron has a wide range of applicability in the pediatric population, is safe and effective in iron deficiency anemia, but requires monitoring and additional future comparative clinical trials.eng
