| dc.description.abstract | Introducción: La congestión venosa es un hallazgo frecuente en el paciente con enfermedad crítica con sepsis asociado a resultados adversos con mayor riesgo de lesión renal aguda y mortalidad. La evaluación mediante VExUS permite la detección y manejo temprano de la congestión venosa Objetivo primario: Medir la prevalencia de congestión venosa por protocolo VExUS en la población de pacientes críticamente enfermos con diagnóstico de sepsis que se tratan en la UCI del HUN. Materiales y métodos: Estudio descriptivo transversal, realizado en pacientes críticos hospitalizados en la UCI del Hospital Universitario Nacional de Colombia entre noviembre de 2024 y julio de 2025. Resultados: Se incluyeron 130 pacientes, la edad media fue 63.6 años, 48.5% fueron de sexo masculino, las comorbilidades más frecuentes fueron hipertensión arterial en 44,6% (n = 58) y diabetes mellitus en 26,9% (n = 35). La congestión venosa detectada por VExUS fue del 55.4% de los cuales el 30.8% presentaron congestión venosa significativa (VExUS ≥ 2) lo cual contrasta con la sobrecarga de fluidos (balance acumulado de líquidos >5% del peso corporal a las 24 horas) que solo fue del 5.4%. La incidencia de lesión renal aguda en los pacientes con congestión severa (VExUS ≥ 2) fue del 72.5%, la estancia media en UCI fue de 11,4 días (DE 11,1) y la mortalidad intrahospitalaria fue de 5,4%. Conclusiones: En esta cohorte, el balance acumulado de líquidos no se asoció con el grado de congestión, lo que sugiere que el seguimiento exclusivo de este parámetro podría ser insuficiente para identificar a los pacientes en riesgo. En contraste, la monitorización temprana con estrategias directas basadas en POCUS, como VExUS, podría aportar información más precisa para la detección y el manejo de la congestión venosa. | dc.description.abstract | Introduction: Venous congestion is a frequent finding in critically ill patients with sepsis, associated with adverse outcomes and a higher risk of acute kidney injury and mortality. Evaluation using VExUS enables early detection and management of venous congestion. Primary objective: To measure the prevalence of venous congestion using the VExUS protocol in the population of critically ill patients diagnosed with sepsis treated in the ICU of the University Hospital of Bogotá (HUN). Materials and methods: A cross-sectional descriptive study was conducted on critically ill patients hospitalized in the ICU of the National University Hospital of Colombia between November 2024 and July 2025. Results: 130 patients were included; the mean age was 63.6 years, and 48.5% were male. The most frequent comorbidities were hypertension in 44.6% (n = 58) and diabetes mellitus in 26.9% (n = 35). Venous congestion detected by VExUS was present in 55.4% of patients, of whom 30.8% presented with significant venous congestion (VExUS ≥ 2). This contrasts with fluid overload (cumulative fluid balance >5% of body weight at 24 hours), which was only 5.4%. The incidence of acute kidney injury in patients with severe congestion (VExUS ≥ 2) was 72.5%, the mean ICU stay was 11.4 days (SD 11.1), and the in-hospital mortality rate was 5.4%. Criterios de Inclusión: Pacientes mayores de 18 años de edad Diagnóstico de sepsis basado en los criterios SEPSIS-3 7.5. Criterios de Exclusión: Pacientes con cirrosis hepática CHILD-PUGH B o mayor. Antecedente de trombosis del sistema venoso portal. Pacientes con ventana ultrasonográfica abdominal inapropiada que impida realizar el protocolo VExUS score. Pacientes con enfermedad renal crónica grave en TRR (tasa de filtración glomerular estimada < 15 ml/min/1.73 mts2 calculada utilizando la fórmula de dieta modificada en enfermedad renal–MDRD- o que se encuentren en terapia de reemplazo renal). 7.6. Tamaño de Muestra y Tipo de Muestreo: Bajo los supuestos de un alfa de 0.05, una potencia del 75%, que se encuentre por lo menos una prevalencia de congestión venosa de cualquier grado (VExUS ≥ 1) en el 20% de la población con una variación de más o menos 10%; el tamaño muestral mínimo se calcula en mínimo 120 - 130 pacientes. Se usó una prueba binomial para estimar el valor de una proporción en relación un numero dado, se usó el software estadístico STATA 15.1 (33–35). Para establecer la diferencia comparativa de los balances de líquidos (Lt) entre los pacientes congestivos y no congestivos se usó una prueba de comparación de dos medias independientes (power twomeans: STATA 15.1); asumiendo una diferencia de por lo menos 0.5 Lt entre los grupos con un poder del 80% y una error alfa de 0.05, además de que por lo menos la prevalencia de congestión fuera del 20%, el tamaño muestral no se altera; es decir se requieren solo 100 pacientes para la comparación. | |
