dc.description.abstractObjetivo: Evaluar la efectividad clínica y la seguridad de las intervenciones farmacológicas y psicológicas utilizadas para el tratamiento del trastorno de ansiedad social en adolescentes. Metodología: Se realizó una revisión sistemática de la literatura con metaanálisis en red, implementando un modelo frecuentista de efectos aleatorios. La reducción de los síntomas de ansiedad social se evaluó como la diferencia estandarizada de medias entre el cambio del puntaje en la línea de base hasta la evaluación de seguimiento. La remisión parcial y completa se evaluaron como desenlace dicotómico a partir del riesgo relativo en la frecuencia del evento. La certeza de la evidencia se evaluó con el enfoque CINeMA. Resultados: Tras la identificación de 9309 referencias en bases de datos y fuentes complementarias se incluyeron 25 ensayos clínicos aleatorizados. La mayoría de los estudios evaluaron diferentes modalidades de la terapia cognitivo-conductual, incluyendo aplicaciones individuales, grupales, virtuales, autodirigidos, entre otros. Entre los tratamientos farmacológicos se identificó el uso de la venlafaxina ER, la fluoxetina y la paroxetina, sin embargo, estos estudios incluyeron población de niños y niñas, y presentaron alto riesgo de sesgo, por lo cual sus resultados fueron limitados. La certeza de la evidencia fue baja para la mayoría de las comparaciones de interés. Los resultados mostraron poca o ninguna diferencia en la reducción de síntomas de ansiedad social y la remisión parcial o completa para adolescentes con trastorno de ansiedad social. Conclusión: Se requiere la generación de más evidencia científica, con mayor rigor metodológico, para determinar las mejores opciones de tratamiento para adolescentes con ansiedad social. dc.description.abstractObjective: To evaluate the clinical effectiveness and safety of pharmacological and psychological interventions used to treat social anxiety disorder in adolescents. Methodology: A systematic literature review with network meta-analysis was performed, implementing a random-effects frequency model. Reduction in social anxiety symptom severity was assessed as the standardized mean difference between the change in the score at baseline to follow-up assessment. Partial and complete remission were assessed as dichotomous outcomes from relative risk in event frequency. The certainty of the evidence was assessed using the CINeMA approach. Results: After identifying 9309 references in databases and complementary sources, 25 randomized clinical trials were included. Most studies evaluated different modalities of cognitive-behavioral therapy, including individual, group, virtual, and self-directed. Among the pharmacological treatments, the use of venlafaxine ER, fluoxetine and paroxetine was identified; however, these studies included a population of children and presented a high risk of bias, so their results were limited. The certainty of the evidence was low for most of the comparisons of interest. The results showed little or no difference in the reduction of social anxiety symptoms and partial or complete remission for adolescents with social anxiety disorder.
