Alimentación complementaria: ¿nuevas tendencias?

dc.description.abstractLa alimentación complementaria es un proceso fundamental para el desarrollo del lactante, tanto desde el punto de vista biológico como social. En esta etapa los niños y niñas se encuentran en mayor riesgo de desnutrición aguda, retraso en talla o exceso de peso por lo que las intervenciones alimentarias son fundamentales para modificar los desenlaces nutricionales a corto y largo plazo. En la última década ha surgido el Baby Led Weaning (BLW), una propuesta alternativa a la alimentación complementaria tradicional con gran aceptación en la comunidad que tiene como esencia la autonomía del lactante y su capacidad de autorregulación y promete mejorar la aceptación alimentaria, regular la ganancia de peso y disminuir las alteraciones metabólicas a largo plazo, por lo que se revisó la literatura publicada con relación a esta estrategia lo que permitió concluir que no hay evidencia de que el BLW genere cambios significativos en cuanto al peso o la composición metabólica, sin embargo sí se asocia a un mayor consumo de vegetales al inicio de la complementaria y a una mayor duración de la lactancia materna; no se ha comprobado un mayor riesgo de atoramiento y con recomendaciones adecuadas no hay aumento del riesgo de ferropenia. Aún son necesarios más estudios que permitan definir si es pertinente la recomendación de la práctica de BLW para la población general. Es importante que el pediatra conozca a profundidad los aspectos biológicos y sociales de la alimentación complementaria y se informe sobre las tendencias recientes de manera que pueda orientar a las familias en este esencial proceso y evitar practicas que puedan resultar nocivas.spa
dc.description.abstractComplementary feeding is a fundamental process in infant development, both biologically and socially. At this stage children are at increased risk of acute malnutrition, stunting and overweight, therefore food interventions are critical to changing nutritional outcomes in the short and long term. In the last decade baby led weaning (BLW) has emerged as an alternative approach to traditional complementary feeding with great acceptance in the community, it is based in the autonomy of the infant and his capacity for self-regulation and promises to improve food acceptance, regulate weight gain and decrease metabolic disturbances in the long term. We reviewed the published literature in complementary feeding and BLW which allowed us to conclude that there is no evidence that the BLW generates significant changes in weight or metabolic composition, however it is associated with increased vegetable consumption at the beginning of complementary feeding and longer duration of breastfeeding; an increased risk of choking has not been proven and with appropriate recommendations there is no increased risk of iron deficiency. Further studies are still needed to define whether the recommendation of BLW practice is relevant and appropriate to the general population. It is important for the pediatrician to know in depth the biological and social aspects of complementary feeding and to be informed of recent trends, to be able to guide families in this essential process and avoid unsafe practices.spa
