Estrategias de tamización de infección por SARS-CoV-2/COVID-19 en pacientes que requieren cirugía electiva: revisión de la literatura

dc.description.abstractIntroducción: La pandemia causada por el virus SARS CoV-2 ha provocado modificaciones ostensibles en el desarrollo de las actividades humanas, incluyendo las del sector salud, como la realización de procedimientos quirúrgicos, debido al mayor riesgo de muerte y de desenlaces adversos de tipo respiratorio cuando estos se realizan a pacientes con infección por SARS CoV-2 tanto sintomática como asintomática. Buscando disminuir este riesgo, han surgido estrategias de tamización de pacientes que requieran cirugía electiva mediante el uso de la RT-PCR como estándar y de otros instrumentos como estudios de imagen y cuestionarios; la presente revisión pretende determinar el rendimiento diagnóstico de dichos instrumentos en comparación con la RT-PCR. Metodología: Se realizó una revisión sistemática de la literatura disponible de febrero 01 de 2020 a marzo 24 de 2021, respecto a la tamización prequirúrgica para infección por SARS CoV-2/COVID-19 entre pacientes requiriendo cirugía electiva. Resultados: A partir de 650 artículos identificados, se revisaron a texto completo 32 documentos y 4 fueron incluidos al cumplir los criterios de elegibilidad. Los estudios analizados incluyeron pacientes programados a procedimientos quirúrgicos no oncológicos de tipo cirugía esencial, electiva y semiurgente de hospitales de EE. UU, Países bajos, Tokio y Japón. La sensibilidad encontrada para la tomografía de tórax fue de máximo un 33%, mientras que para la radiografía de tórax y los cuestionarios estandarizados fue del 50% respecto a RT-PCR. Conclusión: No se encontró evidencia que demuestre que la radiografía de tórax simple, la tomografía de tórax ni los cuestionarios estandarizados, por si solos o usados en conjunto, sean lo suficientemente sensibles para reemplazar a la RT-PCR como instrumento de tamización prequirúrgica para infección asintomática por SARS CoV-2. dc.description.abstractIntroduction: The SARS CoV-2 pandemic has caused remarkable changes in the development of human activities, including those in health services, such as the performing of surgical procedures, due to the increased risk of death and adverse respiratory outcomes when these are performed in patients with both symptomatic and asymptomatic SARS CoV-2 infection. In order to reduce this risk, screening strategies have emerged for patients requiring elective surgery by using the gold standard RT-PCR and other instruments such as imaging studies and questionnaires; the present review aims to determine the diagnostic performance of these instruments in comparison with RT-PCR. Methodology: We performed a systematic review of the literature available from February 01, 2020, to March 24, 2021, regarding pre-surgical screening for SARS CoV-2/COVID-19 infection among patients requiring elective surgery. Results: From 650 identified articles, 32 documents were reviewed in full text and 4 were included when meeting the eligibility criteria. The studies analyzed included patients scheduled for essential, elective, and semi-urgent non-oncological surgical procedures from hospitals in the United States, the Netherlands, Tokyo, and Japan. The sensitivity found for chest tomography was at most of 33%, while for chest radiography and standardized questionnaires it was 50% with respect to RT-PCR. Conclusion: There is no evidence of that plain chest radiography, chest tomography, or standardized questionnaires, alone or used together, are sensitive enough to replace RT-PCR as a presurgical screening tool for asymptomatic infection by SARS CoV-2.eng
