dc.description.abstractEl presente estudio tiene como objetivo establecer los factores pronósticos asociados al desarrollo de muerte intrahospitalaria en pacientes adultos mayores de 65 años con infección por SARS- CoV2/COVID -19 confirmado en el Hospital Universitario Nacional de Colombia. Métodos: Este es un estudio observacional analítico. Se evaluaron pacientes adultos mayores de 65 años con diagnóstico de infección por SARS-CoV2/COVID-19 confirmado. Los pacientes fueron estratificados por estado de fragilidad y funcionalidad. La mortalidad se determinó a los 28 días y se creó un modelo de regresión logística incondicional, ajustando por factores de confusión. La significancia estadística se estableció en p ≤ 0,05. Resultados: 170 pacientes fueron estratificados en sobrevivientes y no sobrevivientes . El porcentaje de hombres en los sobrevivientes y no sobrevivientes fue similar, sin diferencias significativas en la edad promedio entre ambos grupos, tampoco en relación a la presencia de síntomas. En el análisis multivariado, las siguientes variables predijeron la ocurrencia del desenlace mortalidad a 28 días: fragilidad, antecedente de cáncer, SDRA, lesión renal aguda y SOFA. Conclusiones: La tasa mortalidad a 28 días en pacientes adultos mayores de 65 años con infección por COVID 19 en este estudio fue de 38%.Al ajustar por antecedente de cáncer, desarrollo de lesión renal aguda, SDRA, puntuación SOFA y PAFI al ingreso, la fragilidad se asoció con una mayor mortalidad en la población estudiada. dc.description.abstractThe present study aims to establish the prognostic factors associated with the development of in-hospital death in adult patients over 65 years of age with confirmed SARS-CoV2/COVID-19 infection at the National University Hospital of Colombia. Methods: This is an analytical observational study. Adult patients older than 65 years with a confirmed diagnosis of SARS-CoV2/COVID-19 infection was evaluated. The patients were stratified by state of frailty and functionality. Mortality will be reduced at 28 days and an unconditional logistic regression model will be created, adjusting for confounding factors. Statistical significance is set at p ≤ 0.05. Results: There were 170 patients, they were stratified into survivors and non-survivors. The percentage of men in the survivors and non-survivors was similar, without significant differences in the average age between both groups, in relation to the presence of symptoms. In the multivariate analysis, the following variables predicted the incidence of the 28-day mortality outcome: frailty, history of cancer, ARDS, acute kidney injury and SOFA. Conclusions: The 28-day mortality rate in adults older than 65 years with COVID 19 infection in this study was 38%. When adjusting for history of cancer, development of acute kidney injury, ARDS, SOFA score and PAFI at admission, frailty was associated with higher mortality in the study population.eng
