dc.description.abstract | El rechazo agudo del trasplante renal (RAT) es una patología que genera riesgo de pérdida del injerto renal, su diagnóstico es histológico y se basa en el estudio de biopsia usando la clasificación de Banff. Se realizó un estudio analítico con componente descriptivo, de tipo cuantitativo con un análisis observacional retrospectivo de la casuística de pacientes con diagnóstico de RAT en la Fundación Clínica Shaio en los años de 2021 y 2022 haciendo muestreo no probabilístico por conveniencia. Se hizo la descripción y registro fotográfico de los principales hallazgos histológicos de las biopsias usando los criterios de Banff 2019, se buscó asociación entre los tipos de RAT y el donante del injerto renal (Cadavérico/Vivo) mediante el cálculo de Chi-cuadrado. Se encontró una serie de 44 casos de rechazo del injerto agudo que se dividieron en dos grupos, los que presentaron RAT N=35 y otro grupo que presentó RAT Recurrente (RATR) N=8 (un sujeto tuvo 2 episodios de repetición). Se evidenció que el tipo de rechazo de mayor presentación fue el celular tipo IA (62,9%), seguido por celular tipo IB (20%) en el grupo RAT, mientras que, en el grupo RATR se encontró el celular tipo IA (44,4%) seguido por el celular tipo Borderline (22.2%). No se encuentra asociación en estos grupos entre el tipo de rechazo y el origen del injerto, además, las estadísticas confirman que la mayor cantidad de injertos son de donante cadavérico con un 74% en el grupo RAT y del 50% en el grupo RATR (Texto tomado de la fuente). | spa |
dc.description.abstract | Acute renal transplant rejection (ATR) is a pathology that generates risk of renal graft loss, its diagnosis is histological and is based on the biopsy study using the Banff classification. An analytical study was conducted with a descriptive component, quantitative type with a retrospective observational analysis of the casuistry of patients diagnosed with ATR in the Shaio Clinic Foundation in the years 2021 and 2022 with non-probabilistic sampling by convenience. The description and photographic record of the main histological findings of the biopsies was made using the Banff 2019 criteria, association between the types of ATR and the graft donor (Cadaveric/Living) was sought by calculating Chi-square. We found a series of N=44 cases that were divided into two groups, those who presented ATR N=35 and another group that presented Recurrent ATR (RATR) N=8 (one subject had 2 episodes of recurrence). The Banff classification showed that the type of acute graft rejection with the highest presentation was T-cell mediated rejection (TCMR) type IA (62.9%), followed by TCMR type IB (20%) in the ATR group, while in the RATR group, TCMR type IA was found (44.4%) followed by Borderline TCMR (22.2%), no cases of antibody-mediated rejection were found in either group. No association was found in these groups between the type of rejection and the graft donor, in addition, the statistics confirm that the greatest number of grafts are from cadaveric donor with 74% in the ATR group and 50% in the RATR group. | eng |
