Alteraciones en electrogastrografía en gastroparesia: revisión sistemática y metaanálisis

dc.description.abstractAntecedentes: La gastroparesia es un retardo del vaciamiento gástrico sin obstrucción mecánica, en la que hay una pérdida de células intersticiales de Cajal, alterando en las ondas lentas. Mediante la electrogastrografía se evalúan estas ondas lentas. Objetivos: Determinar la prevalencia de alteraciones en los registros de electrogastrografía de niños y adultos con gastroparesia. Metodología: Se realizaron búsquedas sistemáticas en Medline, Embase, LILACS, Web of Science y el Registro Cochrane de Ensayos Controlados. Se incluyeron estudios con gastroparéticos mayores de 6 años evaluados con electrogastrografía. Se evaluó el porcentaje de duración del registro en donde la potencia dominante fue normogastria, taquigastria y bradigastria; la frecuencia dominante; la razón de la potencia; el incremento de la potencia post-estímulo y el coeficiente de inestabilidad de la frecuencia dominante. Se evaluó la calidad metodológica con la herramienta del Instituto Joanna Briggs. Se sintetizaron los datos con resumen narrativo y metaanálisis. Resultados: Se revisaron 3730 artículos, incluyendo 35 artículos, con un total de 1615 pacientes y 355 controles. Al compararlos con controles, los gastroparéticos tenían menos normogastria (ayuno: 50,3% versus 65,8%) (post-estímulo: 54,3%versus 66,5%), más bradigastria (ayuno: 37,7% versus 13%) (post-estímulo: 31,9% versus 16,3%) y más taquigastria (ayuno: 16,1% versus 4,6%) (post-estímulo: 18,3% versus 5,2%). Los gastroparéticos tuvieron menos incremento de la potencia post-estímulo (1,45 dB versus 5,03 dB) y menos razón de la potencia (1,4 versus 5,26). Conclusiones: Alteraciones en la frecuencia y cambios en la amplitud post-estímulo de las ondas lentas, indicando alteraciones en las células intersticiales de Cajal, pueden explicar la gastroparesia. dc.description.abstractBackground: Gastroparesis is a delay in gastric emptying without mechanical obstruction. Loss of interstitial cells of Cajal has been described, altering slow waves. Electrogastrography evaluates gastric slow waves. Aims: Determine the prevalence of alterations in the electrogastrography records of children and adults with gastroparesis. Methodology: Systematic searches were carried out in Medline, Embase, LILACS, Web of Science and the Cochrane Register of Controlled Trials. Studies with gastroparetics older than 6 years studied with electrogastrography were included. The percentage of duration of the recording in which the dominant power is normogastria, tachygastria and bradygastria was evaluated; the dominant frequency; the power ratio; the increase in poststimulus power and the instability coefficient of the dominant frequency. The methodological quality was evaluated with the Joanna Briggs Institute tool. Data were synthesized with narrative summary and meta-analysis. Results: 3730 articles were reviewed, including 35 articles, with a total of 1615 patients and 355 controls.
