Factores asociados a la supervivencia a 5 años y los costos en la atención de pacientes con trasplante renal en Colombia

dc.description.abstractIntroducción: El trasplante renal es la terapia de reemplazo renal ideal para la enfermedad renal crónica avanzada, dado su impacto en la reducción de mortalidad y costos comparado con diálisis. En Colombia es necesario establecer registros propios sobre supervivencia del paciente, costos y factores pronósticos a largo plazo en pacientes post-trasplante renal. Objetivo: Determinar la tasa de supervivencia del paciente, los costos en la atención en salud y sus factores asociados a 5 años en pacientes post-trasplante renal pertenecientes al régimen contributivo en Colombia. Materiales y métodos: Estudio de cohorte retrospectivo utilizando registros administrativos en salud, incluyó 1437 pacientes trasplantados renales pertenecientes al régimen contributivo, desde 1 enero de 2013 hasta 31 diciembre de 2014. Se realizaron análisis para determinar a 5 años: 1) supervivencia del paciente, 2) costos directos desde la perspectiva del tercer pagador y 3) factores asociados; mediante regresiones de Poisson, regresiones lineales y modelos de regresión multivariada multinivel, respectivamente. Resultados y conclusiones: La tasa de supervivencia del paciente a 1 año fue 95.2% y a 5 años 79.82%. Los factores asociados a mayor mortalidad a 5 años fueron la edad (comparado con 18-29): 60-69 años (HR 4.15, [IC 95% 2.39; 7.20], p<0.001), ≥70 años (HR 6.79, [IC 95% 3.51; 13.12], p<0.001); y el índice de comorbilidad de Charlson (ICC) (comparado con ≤3): 4-5 puntos (HR 2.04, [IC 95% 1.56; 2.67], p<0.001), 6-7 puntos (HR 1.54, [IC 95% 1.04; 2.27], p=0.02) y ≥8 puntos (HR 2.25, [IC 95% 1.35; 3.76], p=0.002). La mediana de costos a 5 años postrasplante renal fue 122,059 millones de COP (promedio anual de 24,412 millones de COP). El ICC ≥4 y pertenecer a las regiones geográficas Atlántica, Oriental y Pacífica se asociaron a mayores costos; mientras que ser trasplantado en el 2014 se asoció a menores costos a 5 años en comparación con el año previo. dc.description.abstractIntroduction: Kidney transplantation is the optimal renal replacement therapy for end-stage chronic kidney disease, improving both mortality and costs compared with dialysis. In Colombia, data covering long-term patient survival, health care costs, and related factors are needed in kidney transplant recipients. Objective: Determine patient survival rates, health care costs, and related factors at 5 years in patients with kidney transplant in Colombia. Materials y methods: Retrospective cohort study using administrative health care records, including 1,437 kidney transplant recipient patients from 1 January 2013 to 31 December 2014 included in Colombia´s contributory regime. Analysis were done to determine at 5 years: 1) patient survival, 2) direct health care costs from a third-payer perspective and 3) related factors; using Poisson regressions, linear regressions, and multilevel multivariate regression models, respectively. Results and conclusions: Patient survival rates were 95.2% and 79.82% at 1-year and 5 years, respectively.
