Validación para utilización en Colombia de la escala EORTC QLQ – THY34 para medir la calidad de vida en pacientes con cáncer de tiroides

| dc.description.abstract | El objetivo del estudio fue adaptar y validar el cuestionario EORTC QLQ–THY34 para medir la calidad de vida en pacientes con cáncer de tiroides en población colombiana. Se llevó a cabo un estudió de validación de escalas que incluyó dos metodologías: La Teoría Clásica de los Test (TCT) y el análisis de Rasch. El análisis de TCT incluyó la validez de contenido, consistencia interna, validez de constructo, confiabilidad test retest y sensibilidad al cambio. Se incluyeron 500 participantes con cáncer de tiroides atendidos en el Instituto Nacional de Cancerología. Dentro de los resultados se obtuvo una estructura simplificada de cuatro dominios (Síntomas e impacto, Preocupación relacionada con el cáncer, Alteración orofaríngea y Falta de apoyo social) con adecuadas medidas de bondad de ajuste, valores de consistencia interna satisfactorios y confiabilidad test retest buena para tres dominios y moderada para el cuarto. Se encontraron diferencias significativas en los análisis de sensibilidad al cambio. El análisis de Rasch mostró un adecuado ajuste para la mayoría de los ítems y se encontró un instrumento capaz de medir peores niveles de calidad de vida que la reportada en este estudio. | dc.description.notes | The objective of this study was to adapt and validate the EORTC QLQ-THY34 questionnaire to assess quality of life in patients with thyroid cancer in the Colombian population. A scale validation study was conducted using two approaches: Classical Test Theory (CTT) and Rasch analysis. The CTT analysis included content validity, internal consistency, construct validity, test retest reliability, and sensitivity to change. A total of 500 patients with thyroid cancer treated at the Instituto Nacional de Cancerología (INC) were included. The results showed a simplified four-domain structure (Symptoms and impact, Cancer-related concern, Oropharyngeal alteration, and Lack of social support) with adequate goodness-of-fit indices, satisfactory internal consistency values, and good test-retest reliability for three domains and moderate reliability for the fourth. Significant differences were observed in the sensitivity-to-change analysis. The Rasch analysis demonstrated adequate fit for most items and revealed an instrument capable of measuring lower levels of quality of life than those reported in this study. In conclusion, the EORTC QLQ-THY34 questionnaire, adapted into Colombian Spanish, is a valid and reliable instrument to assess quality of life in patients with thyroid cancer in Colombia. | eng |
