| dc.description.abstract | Introducción: La diabetes mellitus es una patología metabólica heterogénea en donde las complicaciones en los miembros inferiores son frecuentes con un impacto significativo en la morbimortalidad. Metodología: Estudio retrospectivo de cohortes basado en datos administrativos de pacientes mayores de 18 años pertenecientes al régimen contributivo con Diabetes Mellitus en Colombia, que fueron llevados a amputación no traumática de miembros inferiores entre los años 2013 y 2014. El objetivo del estudio es la descripción de características sociodemográficas así como los factores pronósticos asociados a la supervivencia a 5 años. Resultados: Se incluyeron 1843 pacientes, con edad promedio fue de 69 años de edad y el 60,12% fueron hombres. Las áreas geográficas con mayor representación fueron Valle del Cauca (17.85%, n=329), Bogotá (17.31%, n=319) y Antioquia (12.86%, n=237). En el análisis de supervivencia, el 42,87% de los pacientes sobrevivieron durante el seguimiento a 5 años, siendo menor en el género femenino (40%), sin diferencias estadísticamente significativas entre las diferentes regiones del país. En el análisis de regresión multivariada de Cox, la enfermedad renal crónica tuvo una reducción del 58% en la probabilidad de sobrevivir (HR: 1.58; IC 95%: 1.38–1.80; p < 0.001), así como la demencia, la enfermedad pulmonar obstructiva crónica y los eventos cerebrovasculares previos Discusión y conclusión: La amputación en miembros inferiores en pacientes diabéticos se asocia con una supervivencia limitada, determinada principalmente por la edad y la carga de comorbilidad. | dc.description.abstract | Introduction: Diabetes mellitus is a heterogeneous metabolic disease in which lower limb complications are frequent and have a significant impact on morbidity and mortality. Methods: A retrospective cohort study was conducted using administrative data from patients aged ≥18 years enrolled in the contributory health insurance system in Colombia who had diabetes mellitus and underwent non-traumatic lower limb amputation between 2013 and 2014. The objective of the study was to describe sociodemographic characteristics and prognostic factors associated with 5-year survival. Results: A total of 1,843 patients were included, with a mean age of 69 years; 60.12% were male. The regions with the highest representation were Valle del Cauca (17.85%, n= 329), Bogotá (17.31%, n = 319), and Antioquia (12.86%, n = 237). In the survival analysis, 42.87% of patients survived during the 5-year follow-up, with a lower survival rate among females (40%) and no statistically significant differences among regions. In the multivariate Cox regression analysis, chronic kidney disease was associated with a 58% reduction in survival probability (HR: 1.58; 95% CI: 1.38–1.80; p < 0.001), as were dementia, chronic obstructive pulmonary disease, and prior cerebrovascular events. Discussion and Conclusions: Lower limb amputation in patients with diabetes mellitus is associated with poor long-term survival, primarily driven by age and comorbidity burden. These findings underscore the urgent need for comprehensive interventions focused on the prevention and optimal management of cardiovascular and renal comorbidities. | eng |
