dc.description.abstractEl Trastorno por Déficit de Atención e Hiperactividad (TDAH) representa un desafío en la salud pública debido a su impacto en el desempeño escolar, laboral y social de los niños y niñas. Se ha observado que las alteraciones en el eje hipotálamo-hipófisis-adrenal (HHA), especialmente la producción de cortisol, afectan la regulación de los estados de alerta y la reactividad sensorial, contribuyendo al aumento de la severidad de la sintomatología central. Esto genera limitaciones en la participación ocupacional en actividades propias de la niñez, especialmente en el juego. Los tratamientos actuales presentan limitaciones en cuanto a su efectividad y acceso. Por esta razón, se propone la intervención basada en el juego como una alternativa. Esta estrategia, enfocada en activar el eje HHA a través de actividades que estimulan el movimiento y los sentidos, podría ofrecer una solución para mejorar el desempeño ocupacional de los niños con TDAH. Objetivo: Analizar los efectos de una intervención basada en el juego en los niveles salivales matutinos de cortisol y el desempeño ocupacional de la conducta juguetona en una muestra de niños y niñas de 7 a 11 años con TDAH residentes en la ciudad de Bogotá. Métodos: Los instrumentos de evaluación incluyen la Escala de Swanson, Nolan y Pelham (SNAP IV), el Perfil sensorial (PS2), el Test de la conducta juguetona (ToP), la escala de logro de objetivos (GAS) y la prueba Salivette para cortisol. El estudio fue un diseño pre-test post-test cuasi experimental sin grupo control. Resultados: Se observó un cambio significativo en el desempeño ocupacional del juego después de la intervención (p <0.05); sin embargo, no se evidenciaron cambios significativos en los niveles matutinos de cortisol salival. No se logró establecer una correlación entre los resultados de la conducta juguetona, la escala de logro de objetivos y los niveles de saliva de los participantes. Conclusiones: Las intervenciones basadas en juego tienen el potencial de mejorar el desempeño ocupacional de los niños y niñas con TDAH mejorando habilidades socioemocionales que les permiten incrementar su participación en esta ocupación. Aunque los niveles de cortisol no cambiaron significativamente, se sugiere explorar más a fondo los mecanismos subyacentes, para establecer estrategias de intervención que permitan abordar integralmente las necesidades de los niños con TDAH. dc.description.abstractAttention Deficit Hyperactivity Disorder (ADHD) represents a challenge in public health due to its impact on the school, work, and social performance of children. It has been observed that alterations in the hypothalamic-pituitary-adrenal axis (HPA), especially cortisol production, affect the regulation of arousal states and sensory reactivity, contributing to the increased severity of central symptomatology. This generates restrictions on occupational participation in childhood occupations, especially in play. Current treatments have limitations in terms of their effectiveness and access. For this reason, play-based intervention is proposed as an alternative. This strategy, focused on activating the HPA axis through activities that stimulate movement and the senses, could offer a solution to improve the occupational performance of children with ADHD. Objective: To analyze the effects of a play-based intervention on morning salivary cortisol levels and the occupational performance of playfulness in a sample of 7 to 11-year-old children with ADHD residing in the city of Bogotá. Methods: Evaluation instruments include the Swanson, Nolan, and Pelham Scale (SNAP IV), the Sensory Profile (PS2), the Test of Playfulness (ToP), the Goal Attainment Scale (GAS), and the Salivette test for cortisol. The study was a quasi-experimental pre-test and post-test design without a control group. Results: A significant change was observed in the occupational performance of play after the intervention (p < 0.05); however, no significant changes were evidenced in morning salivary cortisol levels. No correlation was established between the results of the Test of Playfulness, the Goal Attainment Scale, and participants' saliva levels. Conclusions: Play-based interventions have the potential to mprove the occupational performance of children with ADHD by enhancing socio-emotional skills that allow them to increase their participation in this occupation. Although cortisol levels did not change significantly, it is suggested to further explore the underlying mechanisms to establish intervention strategies that comprehensively address the needs of children with ADHD.eng
