dc.description.abstractPulmonary hypertension (PH) is a rare pathology, and in the literature, most of the existing evidence focuses on adults, from which data are extrapolated to pediatrics. Within its pathophysiology, it is known that hypobaric hypoxia secondary to high altitude plays an important role in its etiology, especially in children, who can be affected since the prenatal stage. Despite the available treatment, PH implies great morbidity and mortality for the pediatric patient. Given the problem described and considering that there are few biomarkers available in clinical practice with enough evidence for the diagnosis and follow-up of this pathology, Endothelin-1 (ET-1) is highlighted, as a molecule involved in the pathophysiological mechanisms that generate PH and in certain drugs used as the first line of treatment. For this reason, an analytical observational case-control study is proposed, taking into account as cases pediatric patients with pulmonary hypertension who developed the disease at high altitude (> 2500 masl), VS at low altitude (<1000 masl), compared with healthy controls that inhabit both levels. In all the subgroups described, ET-1 will be measured in peripheral venous blood (with prior informed consent) and its levels will be analyzed according to the established study groups and the variables selected to characterize the population and the disease, to provide evidence that supports its usefulness as a diagnostic tool and that contributes to establishing the influence of hypobaric hypoxia on the pathophysiology of the disease.spa
dc.description.abstractLa hipertensión pulmonar (HP) es una patología infrecuente, y en la literatura, la mayoría de evidencia existente se enfoca en adultos, desde la cual se extrapolan datos hacia la pediatría. Dentro de su fisiopatología, se conoce que la hipoxia hipobárica secundaria a la altura juega un papel importante en su etiología, en especial en el niño, que puede verse afectado desde la etapa prenatal. A pesar del tratamiento disponible, la HP implica gran morbimortalidad para el paciente pediátrico. Dado el problema descrito y teniendo en cuenta que son pocos los biomarcadores disponibles en la práctica clínica con suficiente evidencia para el diagnóstico y seguimiento de esta patología, se resalta la Endotelina-1 (ET-1), molécula involucrada en los mecanismos fisiopatológicos que generan la HP y en ciertos medicamentos utilizados como primera línea de tratamiento. Por esto, se propone un estudio de casos-controles de tipo observacional analítico teniendo en cuenta como casos a pacientes pediátricos con hipertensión pulmonar que desarrollaron la enfermedad a gran altura (>2500 msnm), VS a baja altura (<1000 msnm), comparados con controles sanos que habitan en ambos niveles. En todos los subgrupos descritos se realizará la medición de ET-1 en sangre venosa periférica (previo consentimiento informado) y sus niveles se analizarán según los grupos de estudio establecidos y las variables seleccionadas para caracterizar la población y la enfermedad, con el fin de aportar evidencia que soporte su utilidad como herramienta diagnóstica y que contribuya a establecer la influencia de la hipoxia hipobárica en la fisiopatología de la enfermedad.spa
