Concordancia entre ecografía de glándulas lagrimales y salivales e histopatología de glándula salival menor en adultos con síndrome seco del Hospital Universitario Nacional

| dc.description.abstract | Introducción. La enfermedad de Sjögren es una enfermedad autoinmune que afecta principalmente las glándulas exocrinas. Objetivo: conocer la concordancia entre hallazgos ecográficos de glándulas salivales y lagrimales y los resultados histopatológicos de biopsias de glándulas salivales menores en pacientes con síndrome seco atendidos en un hospital de tercer nivel en Colombia. Metodología: Estudio de corte transversal. Se incluyeron 66 pacientes, de los cuales 42 se clasificaron como enfermedad de Sjögren y 24 como síndrome seco no enfermedad de Sjögren, según criterios ACR/EULAR 2016. Se registró el resultado de focus score de la biopsia de glándulas salivales menores y se realizó ecografía glandular. Resultados: Los hallazgos ecográficos mostraron que el 57.6% de los pacientes presentaron anormalidades glandulares, siendo más frecuentes en glándulas submandibulares. La ecografía de glándulas parótidas presentó alta especificidad (100%) para el diagnóstico de enfermedad de Sjögren, pero su sensibilidad fue limitada (24-30%). No se encontró asociación significativa entre anormalidades en glándulas lagrimales y el diagnóstico de enfermedad de Sjögren. La concordancia entre los hallazgos ecográficos y la biopsia de glándulas salivales menores fue escasa (grado de acuerdo: 57%). Conclusión: aunque la ecografía glandular puede ser útil como herramienta complementaria, aún no puede reemplazar la biopsia de glándulas salivales menores en el diagnóstico de enfermedad de Sjögren en nuestro medio. | dc.description.abstract | Introduction: Sjögren's disease is an autoimmune disease that primarily affects the exocrine glands. Objective: To assess the agreement between ultrasound findings of salivary and lacrimal glands and the histopathological results of minor salivary gland biopsies in patients with sicca syndrome seen at a quaternary care hospital in Colombia. Methods: Cross-sectional study. A total of 66 patients were included, of whom 42 were classified as having Sjögren's disease and 24 as having non-Sjögren sicca syndrome, according to the 2016 ACR/EULAR criteria. Minor salivary gland biopsy focus scores were recorded, and glandular ultrasound was performed. Results: Ultrasound findings showed that 57.6% of patients presented glandular abnormalities, most frequently in the submandibular glands. Parotid gland ultrasound had high specificity (100%) for the diagnosis of Sjögren's disease, but limited sensitivity (24–30%). No significant association was found between abnormalities in lacrimal glands and the diagnosis of Sjögren's disease. The agreement between ultrasound findings and minor salivary gland biopsy was low (agreement rate: 57%). Conclusion: Although glandular ultrasound may serve as a complementary tool, it cannot yet replace minor salivary gland biopsy for the diagnosis of Sjögren's disease in our setting.
