dc.description.abstract | Introducción: La epilepsia farmacorresistente se define como el fracaso de 2 fármacos antiepilépticos tolerados y elegidos adecuadamente (1). La epilepsia es un problema neurológico pediátrico frecuente, su tasa de prevalencia oscila entre 4.3 y 9.3 casos por cada 1000 niños, y afecta al 1,2% de la población mundial (2, 3). A pesar del tratamiento farmacológico, en 30 a 40% de los pacientes no se logra un control de las crisis, y, aproximadamente la mitad de estos pacientes se convierten en candidatos para la cirugía de epilepsia (3). Una vez confirmado el diagnóstico, las opciones de tratamiento incluyen medicación, regímenes dietéticos, agentes inmunomoduladores y cirugía. La región cerebral por intervenir quirúrgicamente dependerá de la localización de la zona epiléptica y de su importancia funcional, estos factores se determinan en la evaluación prequirúrgica, para así determinar el tipo de cirugía, la cual dependerá de la causa subyacente (1). Objetivo: Establecer la incidencia de libertad de crisis teniendo en cuenta la clasificación de Engel en pacientes llevados a cirugía de epilepsia en la edad pediátrica en HOMI Fundación Hospital Pediátrico La Misericordia. Metodología: Se realizo un estudio de tipo observacional analítico de tipo cohorte retrospectiva, con pacientes menores de edad que fueron tratados en HOMI Fundación Hospital Pediátrico La Misericordia, con epilepsia farmacorresistente, por el grupo de cirugía de epilepsia, en el periodo comprendido entre el 1 de enero de 2013 al 31 de julio de 2023. Resultados: Entre 2013 y julio de 2024, fueron evaluados 326 pacientes por el programa de cirugía de epilepsia del HOMI Fundación Hospital Pediátrico La Misericordia, y, 154 pacientes fueron llevados a cirugía, de los cuales, 56 pacientes presentaron criterios de exclusión, 3 de ellos por tener antecedente de cirugía extrainstitucional, 6 pacientes por no contar con registros completos en las historias clínicas de la institución, y, 47 pacientes no cumplieron con el seguimiento mínimo de 1 año posterior a la realización de la cirugía. La mediana de seguimiento postoperatorio fue de 40,74 meses. El tipo de cirugía aumentó significativamente la probabilidad de resultado favorable a 6 meses (RR =3,54; IC95% 1,86 - 6,77; p>0,001) y a 12 meses de seguimiento (RR =4; IC95% 1,96 - 8,18; p<0,001). El presentar crisis diarias y requerir manejo con más de tres medicamentos se asociaron a una menor probabilidad de resultado favorable durante el seguimiento. Conclusiones: Los años de vida saludable previos al diagnóstico de epilepsia y el tipo de cirugía realizada (paliativa versus curativa) se identificaron como factores independientes fuertemente asociados a resultados favorables en el tratamiento de la epilepsia farmacorresistente (Texto tomado de la fuente). | spa |
dc.description.abstract | Introduction: Drug-resistant epilepsy is defined as the failure of 2 adequately tolerated and chosen antiepileptic drugs (1). Epilepsy is a common pediatric neurological problem, its prevalence rate ranges between 4.3 and 9.3 cases per 1000 children, and it affects 1.2% of the world's population (2, 3). Despite drug treatment, seizure control is not achieved in 30 to 40% of patients, and approximately half of these patients become candidates for epilepsy surgery (3). Once the diagnosis is confirmed, treatment options include medication, dietary regimens, immunomodulatory agents, and surgery. The brain region to be surgically treated will depend on the location of the epileptic zone and its functional importance; these factors are determined in the presurgical evaluation, in order to determine the type of surgery, which will depend on the underlying cause (1). Objective: To establish the incidence of seizure freedom taking into account the Engel classification in patients undergoing epilepsy surgery in the pediatric age group at HOMI Fundación Hospital Pediátrico La Misericordia. Methodology: An observational, analytical, retrospective cohort study was conducted with minor patients who were treated at HOMI Fundación Hospital Pediátrico La Misericordia, with drug-resistant epilepsy, by the epilepsy surgery group, in the period between January 1, 2013, and July 31, 2023. Results: Between 2013 and July 2024, 326 patients were evaluated by the epilepsy surgery program at HOMI Fundación Hospital Pediátrico La Misericordia, and 154 patients underwent surgery, of which 56 patients met exclusion criteria, 3 of them for having a history of extra-institutional surgery, 6 patients for not having complete records in the institution's medical records, and 47 patients did not comply with the minimum follow-up of 1 year after surgery. The median postoperative follow-up was 40.74 months. The type of surgery significantly increased the probability of a favorable outcome at 6 months (RR = 3.54; 95% CI 1.86 - 6.77; p>0.001) and at 12 months of follow-up (RR = 4; 95% CI 1.96 - 8.18; p<0.001). Presenting daily seizures and requiring management with more than three medications were associated with a lower probability of a favorable outcome in both follow-up periods. Conclusions: Years of healthy life prior to the diagnosis of epilepsy and the type of surgery performed (palliative versus curative) were identified as independent factors strongly associated with favorable outcomes in the treatment of drug-resistant epilepsy. | eng |
