Consenso de expertos para el manejo del cáncer de tiroides en niños

dc.description.abstractIntroducción. El cáncer de tiroides es la neoplasia endocrina más frecuente en la población pediátrica, tiene un comportamiento más agresivo debido a que la tasa de malignidad de los nódulos tiroideos es mayor en niños en comparación con la población adulta y cursa con metástasis local al momento del diagnóstico. En nuestro país no hay protocolos que guíen el tratamiento del cáncer de tiroides en población pediátrica. En la Fundación Hospital Pediátrico la Misericordia (HOMI) hemos identificado un incremento en el número de casos con el paso de los años sin embargo no contamos con un consenso institucional para el manejo de esta patología. Se planteó la elaboración de recomendaciones utilizando la metodología GRADE enfocadas en lograr un diagnóstico temprano y tratamiento adecuado. Metodología. Se formularon tres preguntas siguiendo la metodología PICO, se realizaron revisiones sistemáticas de la literatura para cada pregunta, siguiendo la metodología del manual Cochrane y teniendo en cuenta la importancia de los desenlaces. Se evaluó la calidad de los estudios y se redactaron las recomendaciones. Estas recomendaciones se clasificaron de acuerdo con los criterios trazados por GRADE y fueron validadas por consenso de expertos institucional y externo. Resultados. Las recomendaciones validadas fueron: detección por ultrasonografía (US) de calcificaciones internas y adenopatías cervicales como características de malignidad del nódulo tiroideo, aspiración con aguja fina como estudio de elección en nódulos tiroideos con características clínicas o ecográficas sospechosas; en nódulos tiroideos Bethesda V-VI evaluación preoperatoria por US para valorar el compromiso ganglionar; exploración del compartimiento ganglionar central con monitorización de nervio vago en todos los pacientes y estudio histológico o evaluación intraoperatoria para definir el requerimiento de vaciamiento ganglionar lateral en pacientes con predictores de metástasis ganglionar. Conclusión. El cáncer de tiroides pediátrico es complejo y requiere de un equipo multidisciplinario. La mayoría de las recomendaciones fueron validadas por lo que consideramos que la aplicación de este consenso contribuirá a un diagnóstico asertivo, tratamiento más temprano y apropiado de los pacientes y por ende a una menor morbilidad de los niños afectados por esta patología. dc.description.abstractBackground. Thyroid cancer is the most common endocrine neoplasia in the pediatric population, has a more aggressive behavior because the rate of malignancy of thyroid nodules is higher in children compared to the adult population and it presents with local metastasis at the time of diagnosis. In our country, there are no protocols that guide the treatment of thyroid cancer in the pediatric population. At Fundación Hospital Pediatrico la Misericordia (HOMI) we have identified an increase in the number of cases over the years, however, we do not have an institutional consensus for the management of this pathology. The preparation of qualified recommendations was proposed using the GRADE methodology focused on achieving an early diagnosis and adequate treatment. Methods. Three questions were formulated for the systematic reviews, following the methodology of the Cochrane manual and considering the importance of the outcomes. A summary and qualification of the evidence were made, and the recommendations were written. These recommendations were classified according to the criteria outlined by GRADE and were validated by institutional and external experts. Results. The validated recommendations were: ultrasound detection (US) of internal calcifications and cervical adenopathies as characteristics of malignancy of the thyroid nodule, fine-needle aspiration as the study of choice in thyroid nodules with suspicious clinical or ultrasound characteristics; in Bethesda V-VI thyroid nodules, preoperative ultrasound evaluation to assess for lymph node involvement; exploration of the central lymph node compartment with monitoring of the vagus nerve in all patients and histological study or intraoperative evaluation to define the need for lateral lymphadenectomy in patients with predictors of lymph node metastasis. Conclusion. Pediatric thyroid cancer is complex and requires a multidisciplinary team. Most of the recommendations were validated, so we consider that applying this consensus will contribute to an assertive diagnosis, earlier and more appropriate treatment of patients, and, therefore, lower morbidity of children affected by this pathology.eng
