Diagnóstico por imágenes de las infecciones profundas postesternotomía: revisión sistemática de la literatura

Abstract

Clásicamente, las infecciones que sobrevienen tras una esternotomía se han clasificado en superficiales y profundas. Las infecciones profundas, o mediastinitis postesternotomía, comprenden una complicación relativamente infrecuente (al rededor del 2%) pero que acarrean una mortalidad importante. Usualmente su diagnóstico es clínico pero, en ocasiones, se hace necesario el uso de ayudas diagnósticas para realizar un manejo certero y oportuno. El presente trabajo pretende exponer los resultados de una revisión sistemática de la literatura que intenta responder la pregunta de cuál es el mejor método de diagnóstico por imágenes para la identificación de dicha complicación.

Abstract. Historically, infections that present after a sternotomy have been classified as either deep or superficial. Deep wound infections (e.g. poststernotomy mediastinits) are a relatively rare but serious complication of such procedures, accounting for a great deal of postsurgical deaths. Usually, its diagnosis is made on clinical basis but sometimes, the use of diagnostic tests comes with a great value for a timely and accurate management of these patients. The current paper intends to expose the results of a systematic review of the academic literature around this important subject and will try to answer the question about which is the best diagnostic imaging for the correct identification of poststernotomy mediastinitis.
